Treatment of varicose ulcer with unna boot: Adverse effects arising from lack of knowledge in its maintenance

Resumo: | Objective: To report a clinical case about the inadequate application of inelastic compression therapy (unna boot) and its adverse effects in the treatment of varicose ulcer. Methodology: This is a clinical case report of a descriptive analytical nature, carried out in a program for the treatment of users with venous ulcers and diabetic foot, this being, non-profit and located in the Agreste of Alagoas in the Arapiraca City. The user's permission was acquired through the Free and Informed Consent Term (FICT) according to resolution 196/96. Case report: M.I.C. user, female, 76 years old, diabetic, hypertensive and with venous ulcer in the right lower limb in the anterior tibial region and lateral region of the malleolus. User arrives at the service with signs of inadequate bandage application, causing bruises and blisters due to excessive compression of the limb. Final Considerations: Upon identifying these signs, interventions were carried out, such as suspension of the use of the inelastic bandage so that there is an improvement of the harmful signs caused by the incorrect application of the Unna Boot, health education for the user and companion, as well as contact with the same for weekly monitoring the evolution of the venous ulcer through a photographic record. In this way, there was a significant improvement in the lesion, and an improvement in the user's quality of life. |

Metodología: Se trata de un reporte de caso clínico de carácter analítico descriptivo, realizado en un programa para el tratamiento de usuarios con úlceras venosas y pie diabético, siendo este, sin fines de lucro y ubicado en el agreste de Alagoas en la ciudad de Arapiraca. El permiso del usuario fue adquirido a través del Término de Consentimiento Libre e Informado (TCLI) según resolución 196/96. Caso clínico: usuaria de M.I.C., sexo femenino, 76 años, diabética, hipertensa y con úlcera venosa en miembro inferior derecho en región tibial anterior y región lateral del maléolo. El usuario llega al servicio con signos de aplicación inadecuada del vendaje, causándole hematomas y ampollas por compresión excesiva de la extremidad. Consideraciones Finales: Al identificar estos signos se realizaron intervenciones como suspensión del uso del vendaje inelástico para que haya una mejoría en los signos nocivos ocasionados por la incorrecta aplicación de la bota de unna, educación en salud para el usuario y acompañante, así como el contacto con ellos para el seguimiento semanal de la evolución de las úlceras venosas a través de registros fotográficos. De esta manera, hubo una mejora significativa en la lesión y una mejora en la calidad de vida del usuario.Objetivo: Relatar um caso clínico vivenciado sobre a aplicação inadequada da terapia compressiva inelástica (bota de unna) e seus efeitos adversos ao tratamento da úlcera varicosa. Metodologia: Trata-se de um relato de caso clínico de cunho analítico descritivo, realizado em um programa para tratamento de usuários portadores de úlcera venosa e pé diabético, este sendo, sem fins lucrativos e situado no agreste de alagoas na cidade de Arapiraca. A permissão da usuária foi adquirida através do Termo de Consentimento Livre e Esclarecido (TCLE) conforme resolução 196/96. Relato de caso: Usuária M.I.C., sexo feminino, 76 anos, diabética, hipertensa e portadora de úlcera venosa em membro inferior direito na região tibial anterior e região lateral do maléolo. Usuária chega ao serviço com sinais de aplicação inadequada da bandagem, ocasionando hematomas e bolhas devido a compressão excessiva do membro. Considerações Finais: Ao identificar estes sinais, foram realizadas intervenções, como suspensão do uso da bandagem inelástica para que haja uma melhora dos sinais prejudiciais ocasionados pela má aplicação da bota de unna, educação em saúde para usuária e acompanhante, assim como, contato com elas para acompanhamento semanal da evolução da úlcera venosa por meio do registro fotográfico.
