Comparison between removable complete prosthesis made by conventional and digital methods: systematic review

Resumo: The objective was to compare the performance of removable complete dentures made by the digital technological method with those made by conventional techniques, through a systematic review. A search was carried out in the VHL, Medline/PubMed, EMBASE and Cochrane databases, from 1991 to January 2022, supplemented by manual search. Studies with edentulous patients, who had complete removable dentures made by the digital and analog method and who had at least one of the primary outcomes (degree of satisfaction, quality of denture finish and clinical performance of the dentures) were included. The initial search resulted in 914 articles, of which only 6 studies were considered eligible for the present search. These studies reported a total of 400 complete dentures installed in 193 patients, 160 of whom had conventional dentures and 240 had digital dentures. The age of the participants ranged from 58 to 69 years, with an average follow-up time of 12 months. Regarding the performance of complete dentures, the digital complete denture presented greater advantages compared to the conventional one in relation to: the degree of patient satisfaction; to the average time of execution of complete dentures, where an increase of 3,5 hours more for conventional totals; in terms of cost, being also higher for the manufacture of conventional prostheses. Therefore, it can be concluded that the studies suggest that the performance of the digital complete denture seems to be superior in terms of patient satisfaction, reduced number of clinical sessions and lower laboratory costs when compared to conventional prostheses.

La búsqueda en las bases de datos BVS, Medline/PubMed, EMBASE y Cochrane, desde 1991 hasta enero de 2022, complementada con búsqueda manual. Se incluyeron estudios con pacientes desdentados, que tenían prótesis removibles completas realizadas por el método digital y analógico y que tenían al menos uno de los resultados primarios (grado de satisfacción, calidad del acabado de la prótesis y rendimiento clínico de la prótesis). La búsqueda inicial dio como resultado 914 artículos, de los cuales solo 6 estudios se consideraron elegibles para la presente búsqueda. Estos estudios reportaron un total de 400 prótesis completas instaladas en 193 pacientes, 160 de los cuales tenían prótesis convencionales y 240 tenían prótesis digitales. La edad de los participantes osciló entre 58 y 69 años, con un seguimiento medio de 12 meses. En cuanto al desempeño de las prótesis completas, la prótesis completa digital presentó mayores ventajas frente a la convencional relación a: el grado de satisfacción del paciente; al tiempo medio de ejecución de prótesis completas, donde un aumento de 3,5 horas más para totales convencionales; en cuanto a coste, siendo también superior para la fabricación de prótesis convencionales. Los estudios sugieren que el desempeño de la prótesis completa digital parece ser superior en términos de satisfacción del paciente, menor número de sesiones clínicas y menores costos de laboratorio en comparación con las prótesis convencionales.O objetivo foi de comparar o desempenho das próteses totais removíveis confeccionadas pelo método tecnológico digital com as fabricadas pelas técnicas convencionais, através de uma revisão sistemática. Foi realizada uma pesquisa nos bancos de dados BVS, Medline/Pubmed, EMBASE e Cochrane, de 1991 a janeiro de 2022, complementada por busca manual. Foram incluídos estudos com pacientes desdentados total, o qual obtiveram confecção de prótese totais removíveis confeccionadas pelo método digital e analógico e que apresentaram pelo menos um dos desfechos primários (grau de satisfação, qualidade de acabamento da prótese e desempenho clínico das próteses). A pesquisa inicial resultou em 914 artigos, onde apenas 6 estudos foram considerados elegíveis para a pesquisa. Nesses estudos foram reportado um total de 400 próteses totais instaladas em 193 pacientes, sendo 160 portadores de próteses convencionais e 240 de próteses digitais. A idade dos participantes variou entre 58 à 69 anos, com um tempo médio de acompanhamento de 12 meses. Quanto ao desempenho das próteses totais, a prótese total digital apresentou maiores vantagens em comparação com a convencional em relação: ao grau de satisfação dos pacientes; ao tempo médio de execução das próteses totais, onde foi reportado um acréscimo de 3,5 horas a mais para as totais convencionais; quanto ao custo, sendo também superior para a confecção das próteses convencionais.
