Strategies to minimize perineal trauma during natural childbirth

Resumo: | Trauma to the woman's perineum is often seen in vaginal delivery. Its occurrence depends on factors that are related to maternal and fetal conditions, the birth itself and the practice of episiotomy, thus showing the need for strategies to reduce the chances of perineal trauma occurrences. The objective of this work is to identify which strategies are used to minimize perineal trauma, based on an integrative literature review with a qualitative approach and a descriptive study. As inclusion criteria: articles in Portuguese and English, in a period of 10 years, with abstracts available in selected computerized databases. Fifty-one articles emerged in the literature that responded to the research objective, after applying the eligibility criteria, 10 articles remained. Thus, it was found that the strategies used, when necessary, are associated with fewer perineal lacerations and lower rates of episiotomy. Therefore, the following results were found: Encouraging upright position, spontaneous pushing, encouraging water births, vaginal delivery as a protective factor for perineal laceration and expectant, hands off perineal protection. It was noted that the obstetric practices used by professionals to reduce and/or minimize perineal trauma at childbirth, contributed to increasing the rates of perineal integrity and reducing the number of episiotomy; in order to prevent the physical and psychological damage that can result from complications caused by severe lacerations and episiotomy in the female perineum. However, there is a need for further studies, due to the relevance of the theme for women's health, in addition to contributing to the progress of scientific knowledge. |

Su ocurrencia depende de factores que están relacionados con las condiciones maternas y fetales, el propio parto y la práctica de la episiotomía, mostrando así la necesidad de estrategias para reducir las posibilidades de ocurrencia de trauma perineal. El objetivo de este trabajo es identificar qué estrategias se utilizan para minimizar el trauma perineal, a partir de una revisión integrativa de la literatura con un enfoque cualitativo y un estudio descriptivo. Como criterios de inclusión: artículos en portugués e inglés, en un período de 10 años, con resúmenes disponibles en bases de datos informatizadas seleccionadas. Cincuenta y un artículos surgieron en la literatura que respondieron al objetivo de la investigación, después de aplicar los criterios de elegibilidad, quedaron 10 artículos. Así, se encontró que las estrategias utilizadas, cuando es necesario, se asocian con menos laceraciones perineales y menores tasas de episiotomía. Por lo tanto, se encontraron los siguientes resultados: Fomento de la posición erguida, pujo espontáneo, fomento del parto en el agua, parto vaginal como factor protector para la laceración perineal y protección perineal expectante, sin manos. Se constató que las prácticas obstétricas utilizadas por los profesionales para reducir y/o minimizar el trauma perineal en el parto, contribuyeron para aumentar los índices de integridad perineal y reducir el número de episiotomías; con el fin de prevenir el daño físico y psíquico que pueden resultar de las complicaciones ocasionadas por laceraciones severas y episiotomía en el perineo femenino. Sin embargo, existe la necesidad de más estudios, debido a la relevancia del tema para la salud de la mujer, además de contribuir para el progreso del conocimiento científico.O trauma no períneo da mulher é frequentemente observado no parto vaginal. Sua ocorrência depende de fatores que estão relacionados às condições maternas e fetais, ao parto em si e à prática da episiotomia, mostrando assim, a necessidade de estratégias para reduzir as chances de ocorrências do trauma perineal. Objetiva-se nesse trabalho identificar quais estratégias utilizadas para minimizar os traumatismos perineais, a partir de uma revisão integrativa da literatura com abordagem qualitativa e estudo descritivo. Como critério de inclusão: artigos nos idiomas português e inglês, no período de 10 anos, com resumos disponíveis nos bancos de dados informatizados selecionados. Emergiram na literatura 51 artigos que responderam ao objetivo da pesquisa, após aplicar os critérios de elegibilidade, restaram 10 artigos. Sendo assim, constatou-se, que as estratégias utilizadas, quando necessário, estão associadas a menos lacerações perineais e menores taxas de episiotomia. Diante disso, foram encontrados os seguintes resultados: Incentivo a posição verticalizada, puxos espontâneos, encorajamento dos partos na água, parto vaginal como um fator protetor para laceração no períneo e a proteção perineal expectante, hands off. Notou-se que às práticas obstétricas utilizadas pelos profissionais para reduzir e/ou minimizar o trauma perineal no parto, contribuíram para aumentar as taxas de integridade perineal e reduzir o número de episiotomia; a fim de prevenir os danos físicos e psíquicos que podem advir das complicações causadas pelas lacerações graves e episiotomia no períneo feminino.
