Nursing care for patients with primary immunodeficiency: an integrative review

Resumo: Introduction: The Primary Immunodeficiency Diseases comprise more than 400 disorders of the immune system, are rare and associated with abnormal development of immune system cells. Objective: To analyze how nursing care for patients with primary immunodeficiency in health systems is described in the scientific literature. Method: An integrative literature review that included journals in Portuguese, English or Spanish available in full and free text in the CINAHL, MEDLINE, EMBASE, Web of Science and Virtual Health Library databases, from 2000 to 2021. Abstracts, editorials and other systematic or integrative literature reviews were excluded. Results: Nine publications were selected. Descriptive studies predominated, with the qualitative approach being the most prevalent. Regarding the evaluation performed with the CASP standardized instrument for analyzing articles, approximately 33% of the selected studies were classified as level A. Discussion: Nursing has numerous nursing attributions in this scenario, such as: performing actions before, during and after immunoglobulin infusion; informing and supporting family members/caregivers; be aware of the patient's social context; promote self-care; prevent and control infections related to health care and encourage adherence to treatment, among other attributions. Conclusion: It is possible to acknowledge the need for trained professionals with scientific knowledge to correctly guide and offer quality care to patients with PID. In addition, further studies are needed on the role of nursing in this scenario, aiming at the propagation of knowledge.

Objetivo: Analizar la atención de enfermería a los pacientes con inmunodeficiencia primaria en entornos de salud descrita en la literatura científica. Método: Revisión integrativa de la literatura que incluyó revistas en portugués, inglés o español disponibles de forma completa y gratuita en el CINAHL, MEDLINE, EMBASE, Web of Science y Biblioteca Virtual de Salud, de 2000 a 2021. Se excluyeron resúmenes, editoriales, revisiones de literatura y otras publicaciones sistemáticas o integradoras. Resultados: Se seleccionaron nueve publicaciones. Predominaron los estudios descriptivos, siendo el enfoque cualitativo el más prevalente. En cuanto a la evaluación realizada con el instrumento para el análisis de artículos “CASP”, aproximadamente 33% de los estudios seleccionados fueron clasificados en el nivel A. Discusión: La enfermería tiene numerosas atribuciones de enfermería en este escenario, tales como: realizar acciones antes, durante y después de la infusión de inmunoglobulinas; informar y apoyar a la familia/cuidadores; ser consciente del contexto social del paciente; promover el autocuidado; prevenir y controlar las infecciones relacionadas con la atención de la salud y fomentar la adherencia al tratamiento, entre otras atribuciones. Conclusión: Son necesarios profesionales capacitados con conocimientos científicos para orientar correctamente y ofrecer una atención de calidad a los pacientes con EIP. Además, se necesitan más estudios sobre el papel de la enfermería en este escenario, con el objetivo de la propagación del conocimiento.Introdução: As Doenças de Imunodeficiência Primária compreendem mais de 400 distúrbios do sistema imune, são raras e estão associadas ao desenvolvimento anormal das células do sistema imunológico. Objetivo: Analisar como é descrita na literatura científica a assistência de enfermagem aos pacientes com imunodeficiência primária nos cenários de saúde. Método: Trata-se de uma revisão integrativa da literatura que incluiu periódicos em português, inglês ou espanhol disponíveis na íntegra gratuitamente nas bases de dados CINAHL, MEDLINE, EMBASE, Web of Science e Biblioteca Virtual em Saúde, no período de 2000 a 2021. Excluiu-se resumos, editoriais e outras revisões sistemáticas ou integrativas da literatura. Resultados: Foram selecionadas nove publicações. Estudos do tipo descritivos predominaram, sendo a abordagem qualitativa mais prevalente. No que se refere à avaliação realizada com o instrumento padronizado de análise de artigos CASP, aproximadamente 33% dos estudos selecionados foram classificados como nível A. Discussão: A enfermagem possui inúmeras atribuições da enfermagem nesse cenário, sendo elas: executar ações antes, durante e após a infusão de imunoglobulina; informar e apoiar os familiares/cuidadores; estar atento ao contexto social do paciente; promover o autocuidado; prevenir e controlar infecções relacionadas à assistência à saúde e incentivar a adesão ao tratamento, dentre outras atribuições. Conclusão: É possível perceber a necessidade de profissionais capacitados e com conhecimento científico para orientar corretamente e oferecer um atendimento de qualidade aos pacientes com IDP.
