Denta care in ICU patients: A literature review on the association of the most common diseases caused by poor oral hygiene and the importance of the dentist in prevention

Resumo: Hospital Dentistry is the practice that cares for and prevents oral changes in patients who are hospitalized in Intensive Care Units (ICU) by multidisciplinary teams. Periodontal diseases (PD) are infectious, asymptomatic and chronic, which occur from the exposure of the host to the action of bacteria that settle on the teeth, which may be gingivitis, periodontitis, or another systemic disease, such as Nosocomial Pneumonia (PN) an acute infection of the lungs, related to poor hygiene of the oral cavity, considered the second most common infection in hospitalized patients. The care by dental surgeons (CD) in (ICU) has as main purpose the prevention of oral infections, which can interfere in the evolution of the pathologies installed in the hospitalized patient. In this sense, the article aims to review the integrative literature, seeking to associate periodontal and systemic diseases with the importance of the Dental Surgeon in ICUs. To guide the research, databases were consulted: Google Scholar, SciELO, Pubmed and Lilacs, using the descriptors: “preventive dentistry”, “hospital dentistry”, “dental care in the ICU”, “oral diseases acquired in the ICU”; in Portuguese and English, from 2000 to 2021. The study made it possible to understand that the CD is indispensable in the care of patients affected with both PD and PN. It is concluded that the CD can act on infections of the oral cavity and respiratory tract, controlling microorganisms that colonize the mouth, in adequate hygiene and in medication action, providing a better quality of life for the patient.

As doenças periodontais (DP) são infecciosas, assintomáticas e de caráter crônico, que ocorrem proveniente da exposição do hospedeiro a ação de bactérias que se estabelecem nos tecidos de inserção dos dentes, podendo ser a gengivite, periodontite, ou outra doença sistêmica, como a Pneumonia Nosocomial (PN), infecção aguda dos pulmões, relacionada com a má higienização da cavidade bucal e considerada a segunda infecção mais comum em pacientes hospitalizados. O atendimento por cirurgiões dentistas (CD) em (UTI), tem como principal finalidade a prevenção de infecções bucais, as quais podem interferir na evolução das patologias instaladas no paciente internado. O artigo tem como objetivo fazer uma revisão de literatura integrativa, buscando associar as doenças periodontais e as sistêmicas com a importância do Cirurgião Dentista nas UTIs. Para nortear a pesquisa foram consultadas bases de dados: Google Acadêmico, SciELO, Pubmed e Lilacs, utilizando os descritores: “odontologia preventiva”, “odontologia hospitalar”, “atendimento odontológico na UTI”, “doenças bucais adquiridas em UTI”; em português e inglês, no período de 2000 a 2021. O estudo permitiu, entender que o CD é indispensável no atendimento de pacientes acometidos tanto com as DP, quanto com a PN. Conclui-se que o CD pode atuar nas infecções da cavidade oral e no trato respiratório controlando micro-organismos que colonizam a boca, na higienização adequada e na ação medicamentosa, proporcionando uma melhor qualidade de vida do paciente.La Odontología Hospitalaria es la práctica que atiende y previene alteraciones bucales en pacientes que se encuentran hospitalizados en Unidades de Cuidados Intensivos (UCI) por equipos multidisciplinarios. Las enfermedades periodontales (EP) son infecciosas, asintomáticas y crónicas, que se producen por la exposición del huésped a la acción de bacterias que se asientan en los dientes, que pueden ser gingivitis, periodontitis u otra enfermedad sistémica, como la Neumonía Nosocomial (NP). Una infección aguda de los pulmones, relacionada con la mala higiene de la cavidad oral, considerada la segunda infección más común en pacientes hospitalizados. La atención por parte de los cirujanos dentistas (CD) en (UCI) tiene como finalidad principal la prevención de infecciones orales, que pueden interferir en la evolución de las patologías instaladas en el paciente hospitalizado. En ese sentido, el artículo tiene como objetivo revisar la literatura integradora, buscando asociar las enfermedades periodontales y sistémicas con la importancia del Cirujano Dentista en las UTI. Para orientar la investigación, se consultaron las bases de datos: Google Scholar, SciELO, Pubmed y Lilacs, utilizando los descriptores: “odontología preventiva”, “odontología hospitalaria”, “atención odontológica en UCI”, “enfermedades bucales adquiridas en UCI”; en portugués e inglés, de 2000 a 2021. El estudio permitió comprender que la CD es indispensable en el cuidado de los pacientes afectados tanto por la EP como por la NP.
