Characterization of AMI hospitalization and mortality in pandemic times, analysis of 2018-2022, in Brazil

Resumo: Cardiovascular Diseases (CVD) are the main cause of general mortality in Brazil and in the world, among which the Acute Myocardial Infarction (AMI) stands out, but a new scenario of lethality in patients infected with Sars-cov-02 has become the focus of care for all hospitals worldwide. This study aimed to characterize the hospitalization and mortality due to AMI in pandemic times in the comparison between 2018 and 2022. This is a cross-sectional, descriptive and quantitative study, which used SINAN/DATASUS as a data source. In the studied period, there were 615,145 hospitalizations and 60,134 deaths due to AMI. There was a decline in hospitalization numbers in 2020, with 19.72% re-establishing an increase in 2021, corresponding (23.52%) to maintaining a percentage increase in 2022.  Regarding the sociodemographic profile: 391,747 (63.7%) were male (86.11%) aged > 50 years, white (39.8%) and brown (33.2%). Deaths due to AMI: 2020 (19.07%) and 2021 (22.7%). The sociodemographic profile of deaths is similar to the profile of hospitalizations, the majority being male (56.1%), white and brown race, 38.73% and 31.45%, respectively, age group >70 years (52.48%). It is understood that the impacts of the pandemic may have greater consequences in the post-pandemic period with silent and masked complications, the postponement of preventive medical care, postponements of primary elective surgeries for patients with CVD. It is expected that the results of this research will contribute to the professional improvement of the epidemiological and sociodemographic profile with health actions aimed at the general population and those with greater vulnerability and susceptibility to the disease.

Este estudio tuvo como objetivo caracterizar la hospitalización y la mortalidad por IAM en tiempos de pandemia de 2018 a 2022. Estudio transversal, descriptivo y cuantitativo, que utilizó el SINAN/DATASUS como fuente de datos. En el período estudiado hubo 615.145 hospitalizaciones y 60.134 óbitos por IAM. Hubo una disminución en el número de hospitalizaciones en 2020, con un 19,72% restableciendo un aumento en 2021, lo que corresponde (23,52%) a mantener un aumento porcentual en 2022. En cuanto al perfil sociodemográfico: 391.747 (63,7%) eran del sexo masculino (86,11%) mayores de 50 años, blancos (39,8%) y morenos (33,2%). Muertes por IAM: 2020 (19,07%) y 2021 (22,7%).  El número de muertes es similar al perfil de hospitalizaciones, en su mayoría hombres (56,1%), raza blanca y parda, 38,73% y 31,45%, respectivamente, grupo de edad >70 años (52,48%). Los impactos de la pandemia pueden tener mayores consecuencias en el período pospandemia con complicaciones silenciosas y enmascaradas, como la postergación de la atención médica preventiva, la postergación de cirugías electivas primarias para pacientes con ECV. Se espera que los resultados de esta investigación contribuyan a la mejora profesional del perfil epidemiológico y sociodemográfico con acciones de salud dirigidas a la población en general y aquellos con mayor vulnerabilidad y susceptibilidad a la enfermedad.As Doenças Cardiovasculares (DCV) são a principal causa de mortalidade geral no Brasil e no mundo, dentre elas destaca-se o Infarto Agudo do Miocárdio (IAM), porém um novo cenário de letalidade em pacientes infectados pelo Sars-cov-02 tornou-se o foco de atendimento de todos os hospitais em âmbito mundial. Este estudo objetivou caracterizar o quadro de hospitalização e mortalidade por IAM em tempos pandêmicos no comparativo de 2018 a 2022. Trata-se de um Estudo transversal, descritivo e quantitativo, que utilizou o SINAN/DATASUS como fonte de dados. No período estudado houveram 615.145 hospitalizações e 60.134 óbitos por IAM. Observou-se um declínio dos números de hospitalização no ano de 2020 sendo 19,72% reestabelecendo acréscimo em 2021, correspondendo (23,52%), mantendo percentual de aumento em 2022. Quanto ao perfil sociodemográfico: 391.747 (63,7%) eram do sexo masculino (86,11%) com faixa etária > 50 anos, de raça/cor branca (39,8%) e pardas (33,2%). Óbitos por IAM: 2020 (19,07%) e 2021 (22,7%).  O perfil sociodemográfico dos óbitos se assemelha ao perfil das hospitalizações, maioria no sexo masculino (56,1%), raça branca e parda, 38,73% e 31,45%, respectivamente, faixa etária > 70 anos (52,48%). Compreende-se que os impactos da pandemia podem apresentar reflexos maiores, no período pós pandemia com complicações silenciosas e mascaradas, a postergação da procura médica preventiva, adiamentos de cirurgias eletivas primordiais aos pacientes com DCV.
