Impact of the COVID-19 pandemic on care for patients with acute myocardial infarction

Resumo: Introduction: The pandemic triggered by SARS-CoV-2 brought challenges to the care of patients with AMI, affecting adequate care. In this study, the objective was to carry out an integrative review to analyze the impact of the COVID-19 pandemic on the delay of care for patients with AMI and, consequently, on their prognosis. Methodology: This is an integrative, qualitative literature review with a descriptive approach. A survey of articles was carried out in the main databases using the descriptors “COVID-19”, “Myocardial Infarction” and “STEMI”; and the Boolean operator AND. The inclusion criteria were: original article, available in full for free access, published in 2020 and 2021, in Portuguese, English and/or Spanish. The final sample consisted of 32 articles. Results: Retrospective cohort, retrospective observational study and meta-analyses were used in the article. These studies reported an almost 80% reduction in the admission of patients with AMI. The prognosis was worse during the pandemic period with an increase in the number of complications and an increase in morbidity and mortality. Mortality from AMI increased by 1.3% to 80% in the period of the pandemic when compared to the same period in the previous year pre-pandemic. In addition, some studies found that the mortality of patients with AMI and COVID-19 could increase by up to four times when compared to the patient with COVID alone. Conclusion: There is a reduction in admissions of AMI cases that may be linked to the fear of contamination and social isolation measures that, associated with the overload of the health system, have led to a delay in seeking care.

En este estudio, el objetivo fue realizar una revisión integradora para analizar el impacto de la pandemia de COVID-19 en el retraso de la atención de los pacientes con IAM y, en consecuencia, en su pronóstico. Metodología: Se trata de una revisión bibliográfica cualitativa e integradora con un enfoque descriptivo. Se realizó un levantamiento de artículos en las principales bases de datos utilizando los descriptores “COVID-19”, “Myocardial Infarction” y “STEMI”; y el operador booleano AND. Los criterios de inclusión fueron: artículo original, disponible en su totalidad para acceso gratuito, publicado en 2020 y 2021, en portugués, inglés y/o español. La muestra final estuvo compuesta por 32 artículos. Resultados: En el artículo se utilizaron cohortes retrospectivas, estudios observacionales retrospectivos y metanálisis. Estos estudios reportaron una reducción de casi el 80% en el ingreso de pacientes con IAM. El pronóstico fue peor durante el período pandémico con un aumento del número de complicaciones y un aumento de la morbimortalidad. La mortalidad por IAM aumentó del 1,3% al 80% en el período de la pandemia en comparación con el mismo período del año anterior antes de la pandemia. Además, algunos estudios encontraron que la mortalidad de pacientes con IAM y COVID-19 podría aumentar hasta cuatro veces en comparación con el paciente con COVID solo. Conclusión: Hay una reducción de los ingresos de casos de IAM que puede estar ligada al miedo a la contaminación y a las medidas de aislamiento social que, asociadas a la sobrecarga del sistema de salud, han llevado a la demora en la búsqueda de atención.Introdução: A pandemia desencadeada pelo SARS-CoV-2 trouxe desafios à assistência ao paciente com IAM afetando no atendimento adequado. Metodologia: Trata-se de uma revisão integrativa da literatura, qualitativa, com abordagem descritiva. Foi realizado um levantamento de artigos nas principais bases de dados utilizando os descritores “COVID-19”, “Myocardial Infarction” e “STEMI”; e o operador booleano AND. A mortalidade por IAM aumentou em 1,3% a 80% no período da pandemia quando comparado ao mesmo período do ano anterior pré-pandemia. Além disso, alguns estudos verificaram que a mortalidade de pacientes com IAM e COVID-19 podia aumentar em até quatro vezes quando comparado ao paciente apenas com COVID.
