Resumo: Fibromyalgia is considered an incurable pathology and an idiopathic syndrome because its pathophysiology is not defined. The patient has symptoms such as chronic and generalized pain, tiredness and difficulty sleeping. It is considered frequent and its diagnosis is made by excluding other pathologies. Although there is still no effective treatment, pharmacological therapy helps patients to reduce symptoms. The purpose of this review was to analyze the pathophysiology involved in the development of fibromyalgia, as well as the pharmacological therapies, plants and medicinal compounds currently existing and studied for the treatment of this pathology, considering a period between 2009 and 2020. For that, the Scielo databases, Pubmed and Google Scholar were consulted. Thus, it can be seen that the existing pharmacological therapies mainly cover four classes of drugs, which are the antiepileptic drugs, tricyclic antidepressants, selective serotonin reuptake inhibitors (SSRIs) and noradrenaline reuptake inhibitors, in addition to muscle relaxants, benzodiazepines, analgesic anti-inflammatories and herbal medicines. Despite the several published guidelines regarding the treatment of fibromyalgia, so far there is no fully effective therapy and in view of this the need for new specific drugs for this pathology is evident.

El paciente presenta síntomas como dolor crónico y generalizado, cansancio y dificultad para dormir. Se considera frecuente y su diagnóstico se realiza excluyendo otras patologías. Aunque todavía no existe un tratamiento eficaz, la terapia farmacológica ayuda a los pacientes a reducir los síntomas. El propósito de esta revisión fue analizar la fisiopatología involucrada en el desarrollo de la fibromialgia, así como las terapias farmacológicas, plantas y compuestos medicinales actualmente existentes y estudiados para el tratamiento de esta patología, considerando un período comprendido entre 2009 y 2020. Para ello, las bases de datos de Scielo, Pubmed y Google Scholar fueron consultados. Así, se puede observar que las terapias farmacológicas existentes cubren principalmente cuatro clases de fármacos, que son los fármacos antiepilépticos, los antidepresivos tricíclicos, los inhibidores selectivos de la recaptación de serotonina (ISRS) y los inhibidores de la recaptación de noradrenalina, además de los relajantes musculares, las benzodiacepinas, analgésicos antiinflamatorios y medicinas a base de hierbas. A pesar de las diversas guías publicadas sobre el tratamiento de la fibromialgia, hasta el momento no existe una terapia totalmente eficaz y ante esto se hace evidente la necesidad de nuevos fármacos específicos para esta patología.A fibromialgia é considerada uma patologia incurável e uma síndrome idiopática por não ter sua fisiopatologia definida. O portador apresenta sintomas como dor crônica e generalizada, cansaço e dificuldade no sono. É considerada frequente e seu diagnóstico é realizado através da exclusão de outras patologias. Embora ainda não existe um tratamento efetivo, a terapia farmacológica auxilia os portadores a diminuírem os sintomas. O objetivo dessa revisão foi analisar a fisiopatologia envolvida no desenvolvimento da fibromialgia, bem como as terapias farmacológicas, plantas e compostos medicinais atualmente existentes e estudados para o tratamento desta patologia, considerando um período entre 2009 e 2020. Para tanto, os bancos de dados Scielo, Pubmed e Google Acadêmico foram consultados. Desse modo, pode-se constatar que as terapias farmacológicas existentes abrangem principalmente quatro classes de medicamentos que são as drogas antiepiléticas, antidepressivos tricíclicos, inibidores seletivos da recaptação de serotonina (ISRS) e inibidores da recaptação de serotonina noradrenalina, além de relaxantes musculares, benzodiazepínicos, antiinflamatórios analgésicos e fármacos fitoterápicos.
