Breastfeeding and lactation for infants with congenital heart disease

Resumo: Objective: To explore the data available in the literature on breastfeeding and breastfeeding in infants with heart disease. Methodology: Integrative literature review, searching the SCIELO and PUBMED databases. The inclusion criteria adopted were: belonging to the time frame of the last 5 years, 2017 to 2021, being described in Portuguese, English or Spanish, with full text available. The exclusion criteria were: dealing with the result or effect of surgeries under the specific heart disease in question, editorials, monographs, texts and non-scientific content. Results: 23 studies were selected. Breastfeeding is the most effective option for NB/infant nutrition, as well as a predictor of infection prevention and reduction in NICU length of stay. Infants with heart disease are more susceptible to dysphagia, malnutrition, infections, and early weaning. Conclusion: Health education about the nutritional and immunological properties of breast milk, which are fundamental for reducing the risk of infection, for pre-surgical preparation, as well as for post-surgical recovery, is a very important care for the binomial. The early stimulation of breastfeeding must respect the newborn's clinical condition. The understanding of the team about the importance of the notification of cases of congenital heart disease and the specificities of breastfeeding in babies with heart disease is necessary, so that caregivers are encouraged to invest in breastfeeding and are guided about the risks involved in this process, how to recognize them, and what attitudes to take if intervention is needed.

Metodología: Revisión integrativa de la literatura, consultando las bases de datos SCIELO y PUBMED. Los criterios de inclusión adoptados fueron: pertenecer al período de tiempo de los últimos 5 años, 2017 a 2021, estar descrito en portugués, inglés o español, con texto completo disponible. Los criterios de exclusión fueron: tratarse del resultado o efecto de cirugías bajo la cardiopatía específica de que se trate, editoriales, monografías, textos y contenido no científico. Resultados: se seleccionaron 23 estudios. La lactancia materna es la opción más eficaz para la nutrición del RN/bebé, así como un predictor de prevención de infecciones y reducción de la estancia en la UCIN. Los bebés con enfermedades cardíacas son más susceptibles a la disfagia, la desnutrición, las infecciones y el destete temprano. Conclusión: La educación sanitaria sobre las propiedades nutricionales e inmunológicas de la leche materna, que son fundamentales para reducir el riesgo de infección, tanto para la preparación prequirúrgica como para la recuperación posquirúrgica, es un cuidado muy importante para el binomio. La estimulación temprana de la lactancia materna debe respetar la condición clínica del recién nacido. Es necesaria la comprensión del equipo sobre la importancia de la notificación de los casos de cardiopatías congénitas y las especificidades de la lactancia materna en bebés con cardiopatías, para que los cuidadores se animen a invertir en la lactancia materna y sean orientados sobre los riesgos que implica este proceso, cómo reconocerlos y qué actitudes tomar si se necesita una intervención.Objetivo: Explorar os dados disponíveis na literatura sobre a amamentação e o aleitamento materno em lactentes cardiopatas. Metodologia: Revisão integrativa da literatura, com busca nas bases de dados SCIELO e PUBMED. Os critérios de inclusão adotados foram: pertencer ao recorte temporal dos últimos 5 anos, 2017 a 2021, estar descrito em português, inglês ou espanhol, com disponibilidade na íntegra. Os critérios de exclusão foram: tratar do resultado ou efeito de cirurgias sob a cardiopatia específica em questão, editoriais, monografias, textos e conteúdo não científico. Resultados: Foram selecionados 23 estudos. O aleitamento materno é a opção mais eficaz para nutrição do RN/lactente, assim como é um fator preditor de prevenção de infecções e diminuição no tempo de internamento em UTIN. Bebês cardiopatas estão mais suscetíveis à disfagia, desnutrição, infecções e desmame precoce. Conclusão: Educar em saúde sobre as propriedades nutritivas e imunológicas do leite materno, que são fundamentais para a diminuição do risco infeccioso, para o preparo pré-cirúrgico, bem como para a recuperação pós-cirúrgica, é um cuidado muito importante para o binômio. A estimulação precoce da amamentação deve respeitar a clínica do recém-nascido.
