Transcortical anesthesia in simple and complex tooth extractions

Resumo: Transcortical anesthesia consists of depositing the anesthetic solution in the cancellous bone adjacent to the tooth to be anesthetized. The objective of this work is to present a retrospective scientific survey on the clinical efficacy of transcortical anesthesia applied to simple and complex extractions through a literature review. A search was carried out on the bases Pubmed, SciELO and Scopus, using as models of keywords "intraosseous injection AND oral surgery", "dental extraction AND intraosseous anesthesia", "mandibular third molar surgery AND intraosseous injection" and “tooth extraction AND transcortical anesthesia”. Clinical studies published between 2000 and 2020 in English were included. Laboratory studies, studies in animals, clinical cases and reports of literature were excluded. As for the results, 92 were included using keywords, 7 of which were included after inclusion and exclusion criteria, by reading the titles, abstracts and full texts. The majority (n = 6) were clinical studies, of which 5 were randomized. All studies evaluated the QuickSleeper system, including comparison of transcortical anesthesia with the lower alveolar nerve block technique (n = 3). Pain perception (n = 3) and effectiveness (n=4) were the most recurrent factors in the studies. The transcortical anesthesia is an alternative to conventional techniques and can be applied in cases of failure or insufficiency of conventional anesthesia in simple and complex extractions.

El objetivo de este trabajo es presentar una encuesta científica retrospectiva sobre la eficacia clínica de la anestesia transcortical aplicada a extracciones simples y complejas a través de una revisión de la literatura. En cuanto a la metodología, la investigación se realizó sobre las bases Pubmed, SciELO y Scopus, utilizando como modelos de palabras clave “intraosseous injection AND oral surgery”, “tooth extraction AND intraosseous anesthesia”, “mandibular third molar surgery AND intraosseous injection” y “tooth extraction AND transcortical anesthesia”. Se incluyeron estudios clínicos publicados entre 2000 y 2020 en inglés. Se excluyeron los estudios de laboratorio, los estudios en animales, los casos clínicos y los informes de la literatura. En cuanto a los resultados, se incluyeron 92 mediante palabras clave, 7 de las cuales se incluyeron luego de criterios de inclusión y exclusión, mediante la lectura de títulos, resúmenes y textos completos. La mayoría (n = 6) fueron estudios clínicos, de los cuales 5 fueron aleatorizados. Todos los estudios evaluaron el sistema QuickSleeper, incluida la comparación de la anestesia transcortical con la técnica de bloqueo del nervio alveolar inferior (n = 3). La percepción del dolor (n = 3) y la efectividad (n=4) fueron los factores más recurrentes en los estudios. La anestesia transcortical es una alternativa a las técnicas convencionales y se puede aplicar en casos de falla o insuficiencia de la anestesia convencional en extracciones simples y complejas.Anestesia transcortical consiste no depósito da solução anestésica no osso esponjoso adjacente ao dente a ser anestesiado. O objetivo desse trabalho é apresentar um levantamento retrospectivo científico sobre a eficácia clínica da anestesia transcortical aplicada às exodontias simples e complexa através de revisão de literatura. Foi realizada uma pesquisa nas bases Pubmed, SciELO e Scopus, usando as combinações de palavras-chave “intraosseous injection AND oral surgery”, “tooth extraction AND intraosseous anesthesia”, “mandibular third molar surgery AND intraosseous injection” e “tooth extraction AND transcortical anesthesia”. Foram incluídos estudos clínicos publicados no período de 2000 a 2020, em inglês. Foram excluídos estudos laboratoriais, estudos em animais, casos clínicos e revisões de literatura. Quanto aos resultados, foram localizados 92 aplicando-se as palavras-chave, dos quais 7 foram incluídos após critérios de inclusão e exclusão, através de leitura dos títulos, resumos e textos completos. A maioria (n=6) era estudos clínicos, dos quais 5 eram randomizados. Todos os estudos avaliaram o sistema QuickSleeper, incluindo comparação a anestesia transcortical com a técnica de bloqueio do nervo alveolar inferior (n=3). A percepção dolorosa (n=3) e a eficácia (n=4) foram os fatores mais avaliados nos trabalhos.
