Screening for oral language alteration in children referred to a hearing health service

Resumo: Audiological evaluation is indicated for differential diagnosis purposes in relation to non-linguistic development delays, language development disorders, two-child speech disorders, autism spectrum disorders (ASD), neurological and genetic disorders and/or metabolic. Objective: To track oral language problems in children referred to a hearing health service. Method: 22 clinical videos recorded of subjects aged 2.0 to 4.0 years, 19 males and 03 females, validated in a hearing health service.  The screening instrument “How does your child hear and speak?” was used to pigtail two dice. Two results were analyzed: a descriptive analysis was performed on two data using absolute and relative frequencies. Association analysis was performed using Fisher´s exact test. The median age was 3.1 years. Most children (86,4%) had no hearing loss. Regarding language problems, 77.2% were unable to perform the screening, presenting difficulties in both expressive language (81.8%) and receptive language (72.7%). The subjects with hearing loss (11.8%) had greater difficulties in the receptive aspects (11.8%), and the subjects didn’t achieve expressive language (88.9%). I found that there was no statistically significant association between hearing loss and receptive language variations. Conclusion: The significant number of subjects with evidence of language disorders found in the studied sample deserves attention. The data presented point out the importance of referral for the differential diagnosis of possible language disorders in hearing health services, thus contributing to early intervention.

Objetivo: Realizar un seguimiento de los problemas de lenguaje oral en niños remitidos a un servicio de salud auditiva. Método: 22 videos clínicos grabados de sujetos de 2,0 a 4,0 años, 19 del sexo masculino y 03 del sexo femenino, validado en un servicio de salud auditiva. Se utilizó el instrumento de cribado “¿Cómo oye y habla su hijo?”. Realizó un análisis descriptivo de dos datos utilizando frecuencias absolutas y relativas. El análisis de asociación se realizó mediante la prueba exacta de Fisher. Resultados: La mediana de edad fue de 3,1 años, la mayoría de los niños (86,4%) no tenían pérdida auditiva. En cuanto a los problemas de lenguaje, el 77,2% no pudo realizar el tamizaje, presentando dificultades tanto en el lenguaje expresivo (81,8%) como en el lenguaje receptivo (72,7%). Los sujetos con hipoacusia (11,8%) tuvieron mayores dificultades en los aspectos receptivos (11.8%), y los sujetos no alcanzaron el lenguaje expresivo (88,9%). Encontré que no había una asociación estadísticamente significativa entre la pérdida auditiva y las variaciones del lenguaje expresivo y receptivo. Conclusión: Merece atención el importante número de sujetos con evidencia de trastornos del lenguaje encontrados en la muestra estudiada. Los datos presentados señalan la importancia de la derivación para el diagnóstico diferencial de posibles trastornos del lenguaje en los servicios de salud auditiva, contribuyendo así a la intervención precoz.A avaliação audiológica é indicada para efeitos de diagnóstico diferencial em relação a quadros de atrasos no desenvolvimento da linguagem, transtornos do desenvolvimento da linguagem, transtornos dos sons da fala, transtornos do espectro do autismo (TEA), alterações neurológicas, genéticas e/ou metabólicas. Objetivo: Realizar rastreamento de problemas de linguagem oral em crianças encaminhadas para um serviço de saúde auditiva. Método: 22 vídeos clínicos de sujeitos de 2,0 a 4,0anos, 19 do sexo masculino e 03 do sexo feminino; avaliados em um serviço de saúde auditiva. Foi utilizado o instrumento de triagem “Como o seu filho ouve e fala?” adaptado para a coleta dos dados. Foi realizada a análise descritiva dos dados por meio de frequências absolutas e relativas. A análise de associação foi realizada pelo teste exato de Fisher. Resultados: A média de idade foi de 3.1 anos. A maioria das crianças (86,4%) não apresentou perda auditiva. Em relação aos problemas de linguagem 77,2% falhou na triagem realizada, apresentando dificuldades tanto na linguagem expressiva (81,8%) quanto na linguagem receptiva (72,7%). Os sujeitos com perda auditiva (11,8%) tiveram maiores dificuldades nos aspectos receptivos (11,8%), e os sujeitos não surdos na linguagem expressiva (88,9%). Verificou-se que não houve associação estatisticamente significativa entre perda auditiva e as variáveis da linguagem expressiva e receptiva. Conclusão: A quantidade significativa de sujeitos com indícios de transtornos de linguagem constatada na amostra estudada merece atenção.
