Nosological profile on maternal mortality: the gravity on pregnancy in Marajó II, Pará, Brazil, between 2010 and 2019

Resumo: Objective: To describe the nosological profile of maternal mortality in the Health Region of Marajó II / PA, from 2010 to 2019. Methods: Epidemiological, descriptive, retrospective study, using secondary data from DATASUS, linked to maternal death records, considering the variables: type of delivery, basic cause and time of death, parity and number of prenatal consultations of the mother, in addition to age, marital status, education and occupation at delivery (death situation). Results: No evaluated period in Pará was recorded in 1,061 maternal deaths, representing the ratio of maternal mortality to 113.76 deaths / 100 thousand live births. Of all registered deaths, 71 occurred in Marajó II (98.97 deaths per 100,000 live births). Mortality the main groups, classified by codes of chapter XV of the ICD-10, were: Eclampsia (O15) and Placental abruption (O45). Most of the negative events occurred among young, single pregnant women, with low educational level and without paid activities. Conclusion: The maternal mortality ratio is above acceptable parameters and affects mainly women in situations of high vulnerability. This highlights the need to adopt a new model of health care for women capable of organizing a care network meeting the needs of groups that are economically and socially disadvantaged.

Métodos: Estudio epidemiológico, descriptivo, retrospectivo, utilizando datos secundarios de DATASUS, vinculados a los registros de defunción materna, considerando las variables: tipo de parto, causa básica y hora de muerte, paridad y número de consultas prenatales de la madre, además de edad, estado civil, educación y ocupación al momento del parto (situación de muerte). Resultados: No se registró ningún período evaluado en Pará en 1.061 defunciones maternas, lo que representa la razón de mortalidad materna a 113,76 defunciones / 100 mil nacidos vivos. Del total de defunciones registradas, 71 ocurrieron en Marajó II (98,97 defunciones por cada 100.000 nacidos vivos). Mortalidad los principales grupos, clasificados por códigos del capítulo XV de la CIE-10, fueron: Eclampsia (O15) y Desprendimiento de placenta (O45). La mayoría de los eventos negativos ocurrieron entre mujeres jóvenes embarazadas solteras, con bajo nivel educativo y sin actividades remuneradas. Conclusión: La tasa de mortalidad materna se encuentra por encima de los parámetros aceptables y afecta principalmente a mujeres en situaciones de alta vulnerabilidad. Esto pone de relieve la necesidad de adoptar un nuevo modelo de atención de la salud de la mujer capaz de organizar una red de atención que responda a las necesidades de los colectivos en desventaja económica y social.Objetivo: Descrever o perfil nosológico da mortalidade materna na Região de Saúde do Marajó II/PA, período entre 2010 a 2019. Métodos: Estudo epidemiológico, descritivo, retrospectivo, utilizando dados secundários do DATASUS, vinculado aos registros de óbitos maternos, considerando as variáveis: tipo de parto, causa básica e momento que ocorreu o óbito, paridade e número de consultas pré-natal da mãe, além de idade, estado civil, escolaridade e ocupação quando do parto (situação do óbito). Resultados: No período avaliado foram registrados no Pará 1.061 óbitos maternos, representando razão de mortalidade materna de calculada em 113,76 óbitos/100 mil nascidos vivos. De todos os óbitos registrados, 71 ocorreram no Marajó II (98,97 óbitos a cada 100 mil nascidos vivos). A mortalidade conforme os principais grupos, classificados por códigos do capítulo XV do CID-10, foram: Eclampsia (O15) e Descolamento prematuro da placenta (O45). A maior parte dos eventos negativos ocorreu entre gestantes jovens, solteiras, com baixo grau de escolaridade e sem atividades remuneradas. Conclusão: A razão de mortalidade materna está acima dos parâmetros aceitáveis e afeta principalmente mulheres em situação de alta vulnerabilidade.
