Knowledge assessment about HIV: a scaled-down based Item Response Theory

Resumo: Objective: Based on the Item Response Theory, to elaborate a reduced scale, from the Questionnaire on Knowledge of the Human Immunodeficiency Virus, in the Deprived of Liberty Population. Methodology: Validation study with 571 men and women deprived of their liberty, closed regime, from the 5th Regional Penitentiary South Police Station. By Self-Applied Human Immunodeficiency Virus Knowledge Questionnaire. Analyzed by Item Response Theory, dimensionality by exploratory and confirmatory factor analysis. Internal consistency assessed by Crombach's alpha and the relevance of each item calculated by point biserial correlation. The two-parameter logistic model and identification of the anchor items, according to the level of theta knowledge, made it possible to organize the reduced scale. RESULTS: 571 respondents, 90% men, mean age 32 years, 63% white skin, 61%, no partner and mean schooling 7 years. The original scale items (n=43) with the best psychometric fit were kept. 4 items excluded. The final scale consisted of 39 items, four levels of knowledge about Human Immunodeficiency Virus, positioned in the 45th to 60th percentile range. Of these, a greater number of items was obtained at level 50 (average), suggesting that the instrument is suitable for evaluating individuals with average knowledge on the subject. Conclusion: The reduced scale of the questionnaire on Knowledge of the Human Immunodeficiency Virus, with a one-dimensional model, remained with 39 dichotomous items, with discriminative ages and with median difficulty for the population studied.

Metodología: Estudio de validación con 571 hombres y mujeres privados de libertad, régimen cerrado, de la 5ª Comisaría Penitenciaria Regional Sur. Mediante el Cuestionario de Conocimientos sobre el Virus de la Inmunodeficiencia Humana Autoaplicado. Analizado por Teoría de Respuesta al Ítem, dimensionalidad por análisis factorial exploratorio y confirmatorio. Consistencia interna evaluada por el alfa de Crombach y la relevancia de cada ítem calculada por correlación biserial puntual. El modelo logístico de dos parámetros y la identificación de los ítems ancla, según el nivel de conocimiento theta, permitieron organizar la escala reducida. Resultados: 571 encuestados, 90% hombres, edad media 32 años, 63% piel blanca, 61%, sin pareja y escolaridad media 7 años. Se mantuvieron los ítems de la escala original (n=43) con mejor ajuste psicométrico, 4 artículos excluidos. Escala final estuvo conformada por 39 ítems, cuatro niveles de conocimiento sobre el Virus de la Inmunodeficiencia Humana, posicionados en el rango percentil 45 al 60. De estos, la mayor cantidad de ítems se obtuvo en el nivel 50 (promedio), lo que sugiere que el instrumento es adecuado para evaluando a individuos con conocimientos medios sobre el tema. Conclusión: La escala reducida del cuestionario sobre Conocimiento del Virus de la Inmunodeficiencia Humana, con modelo unidimensional, quedó con 39 ítems dicotómicos, con edades discriminatorias y con mediana dificultad para la población estudiada.Objetivo: Baseado na Teoria de Resposta ao Item, elaborar escala reduzida, a partir do Questionário sobre Conhecimento do Vírus da Imunodeficiência Humana, na População Privada de Liberdade. Metodologia: Estudo de validação com 571 homens e mulheres privados de liberdade, regime fechado, da 5ª Delegacia Penitenciária Regional Sul. Por Questionário sobre Conhecimento do Vírus da Imunodeficiência Humana Auto Aplicado. Analisada pela Teoria de Resposta ao Item, dimensionalidade pela análise fatorial exploratória e confirmatória. Consistência interna avaliada pela alfa de Crombach e a relevância de cada item calculada pela correlação ponto bisserial. O Modelo logístico de dois parâmetros e identificação dos itens âncora, segundo o nível do conhecimento theta, permitiram organizar a escala reduzida. Resultados: 571 respondentes, 90% homens, idade média 32 anos, 63% pele branca, 61%, sem companheiro e escolaridade média 7 anos. Os itens da escala original (n=43) com o melhor ajuste psicométrico foram mantidos. Excluídos 4 itens. Escala final constou 39 itens, quatro níveis de conhecimento sobre Vírus da Imunodeficiência Humana, posicionados no intervalo do percentil 45 a 60. Destes, obteve-se maior número de itens no nível 50 (média), sugerindo que o instrumento é adequado para avaliar indivíduos com conhecimento médio sobre o tema.
