The challenges and responsibilities of the intermunicipal consortium of the 10th regional health of Paraná in the organization and offering of specializes care

Resumo: The Intermunicipal Health Consortium of the West of Paraná (CISOP) was created to supply the low availability of specialized consultations caused by the lack of human resources to work in specialized centers. Thus, this work aims to analyze the functionality of specialized outpatient care of the 10th Health Regional, its integration with primary care and its main limitations. Methodology: The study comprised one physician from each area of ​​the main specialties of CISOP - psychiatry, orthopedics, endocrinology, cardiology, rheumatology, gynecology, general surgery, nephrology, neurology, pulmonology, urology, otolaryngology and pediatric surgeon. The choice of these specialties was due to the greater demand. All respondents answered a semi-structured questionnaire. Results: The results demonstrate that the waiting time for referral from primary health care to CISOP is 7.3 months, while the return time is 6.3 months. When questioning about the formation of the consortium, it is unanimous that the 10th regional contributed to strengthen the regional health system, 76.9% believe that the junction of municipalities provided scalability of care, as well as 92.3% believe in increased purchasing power. Therefore, most agree with the burden of responsibility, system slowness, in addition to accessibility issues and reference errors. Conclusion: the study showed that the CISOP emerges as a regional reinforcement tool for the expansion of the offer and specialized outpatient care, gain of scale in the provision of services. Although there are still challenges that demand a careful look.

Así, este trabajo tiene como objetivo analizar la funcionalidad de la atención ambulatoria especializada de la 10ª Regional de Salud, su integración con la atención primaria y sus principales limitaciones. Metodología: El estudio estuvo compuesto por un médico de cada área de las principales especialidades del CISOP - psiquiatría, ortopedia, endocrinología, cardiología, reumatología, ginecología, cirugía general, nefrología, neurología, neumología, urología, otorrinolaringología y cirujano pediátrico. La elección de estas especialidades se debió a la mayor demanda. Todos los encuestados respondieron un cuestionario semiestructurado. Resultados: Los resultados demuestran que el tiempo de espera para la derivación desde la atención primaria de salud al CISOP es de 7,3 meses, mientras que el tiempo de retorno es de 6,3 meses. Al cuestionar sobre la formación del consorcio, es unánime que la 10ª regional contribuyó para fortalecer el sistema regional de salud, el 76,9% cree que la unión de municipios proporcionó escalabilidad de la atención, así como el 92,3% cree en el aumento del poder adquisitivo . Por lo tanto, la mayoría está de acuerdo con la carga de responsabilidad, la lentitud del sistema, además de los problemas de accesibilidad y los errores de referencia. Ao questionar sobre a formação do consórcio   é unanime que a 10a regional contribuiu para reforçar o sistema regional de saúde, 76,9 % entende que a junção dos municípios proporcionou escalabilidade de atendimento, assim como 92,3% acredita no aumento do poder de compra. Por conseguinte, a maioria está de acordo com a sobrecarga de responsabilidade, lentidão do sistema, além dos problemas de acessibilidade e erros de referência. Conclusão: o estudo demonstrou que o CISOP surge como uma ferramenta de reforço regional para a ampliação da oferta e cuidado ambulatorial especializado, ganho de escala nas prestações de serviços.
