Syphilis in pregnant women: a comparative analysis of the North region of Brazil in the period 2016 to 2018

Resumo: Objective: Describe the incidence and clinical-epidemiological profile of gestational syphilis in the northern region of Brazil in the period from 2016 to 2018. Methodology: Descriptive epidemiological study with a quantitative approach, using secondary data required in the Notifiable Diseases Information System (SINAN) of the SUS Computer Department (DATASUS). The numbers of confirmed cases of syphilis in pregnancy in Northern Brazil were used as well as detection rates (per 1,000 live births) corresponding to the 2016-2018 triennium. Results: 14,307 cases of syphilis were reported in pregnant women in the northern region of Brazil between the years 2016 and 2018, with the highest detection rate during the three years of the series in the state of Acre. In addition, the North region has syphilis detection in pregnant women below the national average, occupying the fourth place in the ranking in 2018. Most pregnant women were aged between 20 and 29 years old, schooling from 5th to 8th grade incomplete, with mixed race, were diagnosed in the third trimester of pregnancy, with primary clinical classification and received treatment with penicillin. Conclusion: Syphilis is reported as a reemerging disease, representing a serious public health problem in Brazilian territory, as evidenced by the substantial increase in the number of cases and in the rates of detection of syphilis in pregnant women observed during the three years of study in the region North. Therefore, it becomes substantial to adopt measures to interrupt the transmission chain and reduce the impact of syphilis.

(SINAN) del Departamento de Informática del SUS (DATASUS). Se utilizaron las cifras de casos confirmados de sífilis en el embarazo en el norte de Brasil y las tasas de detección (por cada 1.000 nacidos vivos) correspondientes al trienio 2016-2018. Resultados: Se notificaron 14.307 casos de sífilis en mujeres embarazadas en la región norte de Brasil entre los años 2016 a 2018, con la mayor tasa de detección durante los tres años de la serie en el estado de Acre. Además, la región Norte presentó detección de sífilis en gestantes por debajo del promedio nacional, ocupando el cuarto lugar en el ranking en el año 2018. La mayoría de las mujeres embarazadas tenían entre 20 y 29 años, escolaridad de 5o a 8o grado incompleta, color marrón, fueron diagnosticadas en el tercer trimestre de gestación, con clasificación clínica primaria y recibieron tratamiento con penicilina. Conclusión: La sífilis se caracteriza como una enfermedad reemergente, que representa un grave problema de salud pública en el territorio brasileño, como lo demuestra el aumento sustancial en el número de casos y en las tasas de detección de sífilis en gestantes observado durante los tres años de estudio en la región. Norte. Por tanto, es importante adoptar medidas encaminadas a interrumpir la cadena de transmisión y reducir la incidencia de la sífilis.Objetivo: Descrever a incidência e o perfil clínico-epidemiológico de sífilis gestacional na região Norte do Brasil no período de 2016 a 2018. Metodologia: Estudo epidemiológico descritivo com abordagem quantitativa, feito através de dados secundários obtidos no Sistema de Informação de Agravos de Notificação (SINAN) do Departamento de Informática do SUS (DATASUS). Utilizaram-se os números de casos confirmados de sífilis na gestação no Norte do Brasil e as taxas de detecção (por 1.000 nascidos vivos) correspondentes ao triênio 2016-2018. Resultados: Foram notificados 14.307 casos de sífilis em gestantes na região Norte do Brasil entre os anos de 2016 a 2018, com a maior taxa de detecção durante os três anos da série no estado do Acre. Ademais, a região Norte apresentou detecção de sífilis em gestantes abaixo da média nacional, ocupando o quarto lugar do ranking no ano de 2018. A maioria das gestantes possuíam idade na faixa etária de 20 a 29 anos, escolaridade de 5ª a 8ª série incompleta, cor parda, obtiveram diagnóstico no terceiro trimestre da gestação, com classificação clínica primária e receberam tratamento com penicilina. Conclusão: A sífilis se caracteriza como uma doença reemergente, representando um grave problema de saúde pública no território brasileiro, como comprovado pelo acréscimo substancial no número de casos e nas taxas de detecção de sífilis em gestantes observados no decorrer dos três anos de estudo na região Norte.
