Possible pathophysiological hypotheses of fibromyalgia: an integrative literature review

Resumo: | Introduction: Fibromyalgia Syndrome (FMS), or simply fibromyalgia, is a chronic pain syndrome characterized by generalized pain, tender points, sleep disturbances and extreme fatigue. Objective: to highlight the etiophysiopathogenic hypotheses of fibromyalgia. Methodology: This is a descriptive research of the integrative literature review type. The research was carried out through online access to the National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) and EBSCO databases. Information Services, March 2022. Results and Discussion: Patients with FMS, which is characterized as a pain amplification syndrome, are extremely sensitive to both painful and non-painful stimuli, including touch, heat and cold, and stimuli mechanics. However, it should be noted that the hypersensitivity of these patients is not limited to pain, but also includes light, sound and olfactory stimuli. The cause of this exaggerated sensitivity of FMS patients has not been determined, but several studies have shown abnormalities of pain processing in the CNS. Most of these studies showed central sensitization, indicating a reduction in the nociceptive threshold in the dorsal horn of the spinal cord and in the brain. Although afferent nociceptive stimuli are necessary for the pain of FMS patients, an important feature of sensitization is that very little sustained nociceptive stimulation in peripheral tissues is required for the maintenance of the sensitized state and chronic pain. Final considerations: The etiology of fibromyalgia is still not well understood. Its cause is believed to be essentially multifactorial, with genetic and psychological aspects playing a role in the development of the disease. |

Objetivo: destacar las hipótesis etifisiopatógenas de la fibromialgia. Metodología: Se trata de una investigación descriptiva del tipo revisión integrativa de la literatura. La investigación se llevó a cabo a través del acceso en línea a la Biblioteca Nacional de Medicina (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) y las bases de datos EBSCO. Servicios, marzo de 2022. Resultados y discusión: los pacientes con FMS, que se caracteriza por un síndrome de amplificación del dolor, son extremadamente sensibles a los estímulos dolorosos y no dolorosos, incluidos el tacto, el calor y el frío, y los estímulos mecánicos. Sin embargo, cabe señalar que la hipersensibilidad de estos pacientes no se limita al dolor, sino que también incluye estímulos lumínicos, sonoros y olfativos. Objetivo: evidenciar as hipóteses etiofisiopatogênicas da fibromialgia. Metodologia: Trata-se de uma pesquisa descritiva do tipo revisão integrativa da literatura. A pesquisa foi realizada através do acesso online nas bases de dados National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Biblioteca Virtual em Saúde (BVS) e EBSCO Information Services, no mês de março de 2022. Resultados e discussão:Os pacientes com SFM, que se caracteriza por ser uma síndrome de amplificação da dor, são extremamente sensíveis aos estímulos dolorosos e não-dolorosos, inclusive toque, calor e frio e estímulos mecânicos. Entretanto, deve-se salientar que a hipersensibilidade desses pacientes não se limita à dor, mas também inclui os estímulos luminosos, sonoros e olfatórios. A causa dessa sensibilidade exagerada dos pacientes com SFM não foi determinada, mas vários estudos mostraram anormalidades do processamento da dor no SNC. A maioria desses estudos evidenciou sensibilização central, indicando redução do limiar nociceptivo no corno dorsal da medula espinhal e no encéfalo. Embora os estímulos nociceptivos aferentes sejam necessários para a dor dos pacientes com SFM, uma característica importante da sensibilização é que se necessita de pouquíssima estimulação nociceptiva sustentada nos tecidos periféricos para a manutenção do estado sensibilizado e dor crônica. Considerações finais: A etiologia da fibromialgia ainda não é bem conhecida.
