Palliative oncology care in the training of Nurses: reflections from the perspective of the concepts of institution, instituted and instituting Institutional Analysis

Resumo: | This article aims to expand the reflections on the teaching of nurses in palliative oncology care from the perspective of the concepts of institution, instituted and instituting Institutional Analysis, considering nursing education as an institution in constant movement and being crossed by other institutions. The research is bibliographic, exploratory and with a qualitative approach, using the theoretical-methodological framework of Institutional Analysis and its concepts of institution, instituted and instituting. The bibliographic survey was carried out in two stages: the first carried out on the Virtual Health Library portal and the second on official documents that address palliative care, which made it possible to expand the discussion on the theme. The results highlight the need for changes in the training of nurses, recognizing that the consolidation of new practices brought about by palliative care constitutes a challenge in the instituted training scenarios, either in the training centers or in the services. It also brings the reflection that instituting processes, such as the nursing practice in palliative care, produce strangeness and ruptures, having as a positive aspect the provocation of changes and expansion of the possibilities of training centers and services to carry out new experiments, problematizing the context where are inserted. It concludes considering that every instituting process must indicate movements of change that lead to new paths, thought by a collective, where professionals and students must perceive themselves as protagonists of this process of building new practices in nursing care. |

La investigación es bibliográfica, exploratoria y con un enfoque cualitativo, utilizando el marco teórico-metodológico del Análisis Institucional y sus conceptos de institución, instituido e instituyente. La encuesta bibliográfica se realizó en dos etapas: la primera en el portal de la Biblioteca Virtual en Salud y la segunda en documentos oficiales que abordan los cuidados paliativos, lo que permitió ampliar la discusión sobre el tema. Los resultados destacan la necesidad de cambios en la capacitación de las enfermeras, reconociendo que la consolidación de nuevas prácticas generadas por los cuidados paliativos constituye un desafío en los escenarios de capacitación instituidos, ya sea en los centros de capacitación o en los servicios. También trae la reflexión de que instituir procesos, como la práctica de enfermería en cuidados paliativos, produce extrañeza y rupturas, teniendo como aspecto positivo la provocación de cambios y la expansión de las posibilidades de los centros y servicios de capacitación para llevar a cabo nuevos experimentos, problematizando el contexto donde están insertados. Concluye considerando que todo proceso de institución debe indicar movimientos de cambio que conduzcan a nuevos caminos, pensados por un colectivo, donde los profesionales y los estudiantes deben percibirse como protagonistas de este proceso de construcción de nuevas prácticas en el cuidado de enfermería.Este artigo objetiva ampliar as reflexões sobre o ensino de enfermeiros em cuidados paliativos oncológicos sob a ótica dos conceitos de instituição, instituído e instituinte da Análise Institucional, considerando-se a formação em enfermagem como uma instituição em constante movimento e sendo atravessada por outras instituições. A pesquisa é bibliográfica, exploratória e com abordagem qualitativa, utilizando o referencial teórico-metodológico da Análise Institucional e seus conceitos de instituição, instituído e instituinte. O levantamento bibliográfico foi realizado em duas etapas: a primeira realizada no portal Biblioteca Virtual em Saúde e a segunda em documentos oficiais que abordam os cuidados paliativos, o que possibilitou ampliar a discussão sobre a temática. Os resultados destacam a necessidade de mudanças na formação de enfermeiros, reconhecendo que a consolidação de novas práticas trazidas pelos cuidados paliativos constitui-se em um desafio nos cenários instituídos de formação, seja nos centros formadores ou nos serviços. Traz ainda a reflexão de que processos instituintes, tais como a prática de enfermagem nos cuidados paliativos, produzem estranhamentos e rupturas, tendo como aspecto positivo a provocação de mudanças e ampliação das possibilidades de centros formadores e serviços para realizarem novas experimentações, problematizando o contexto onde estão inseridas.
