Syphilis in pregnant women: epidemiological and spatial analysis in a state in the Amazon

Resumo: The objective of this study is to describe the epidemiological profile of reported cases of gestational syphilis in the State of Amapá in the five-year period from 2014 to 2018 and to associate the sociodemographic and clinical variables found in the research, and to evaluate the spatio-temporal distribution of the disease in the counties through data plotted on maps using the SIG. Retrospective cross-sectional epidemiological study, with a quantitative nature and descriptive approach. Dice on syphilis notifications in pregnant women were taken from the Notifiable Diseases Information System (SINAN) and the number of born alive from the Alive Born Information System (SINASC), and the geographic meshes of the Brazilian Institute of Geography and Statistics (IBGE). From 2014 to 2018, 986 cases of syphilis were registered in pregnant women, with a number bigger in the City of Macapá (646) and a higher incidence in Serra do Navio (26.55). Most of the notifications occurred in the age group of 20 to 29 years (47.7%), the brown race (74.75%), pregnant women with incomplete middle school (22.21%), housewives (58.82%), third trimester of pregnancy (50.71%) and the classification of the disease in the primary phase (27.28%). The prevailing treatment was benzathine Penicillin G 7,200,000 IU (49.39%). The findings indicated an annual increase in syphilis notifications during pregnancy, as well as considerable growth in the annual incidence coefficient in the counties of the State of Amapá, revealing the need to interrupt the syphilis transmission chain.

Estudio epidemiológico transversal, retrospectivo, con abordaje cuantitativo y descriptivo. Los dados de sífilis en gestantes se tomó del Sistema de Información de Enfermedades Notificables (SINAN), el número de nacidos vivos del Sistema de Información de Nacimientos Vivos (SINASC) y las mallas geográficas del Instituto Brasileño de Geografía y Estadística (IBGE). Entre 2014 y 2018 se registraron 986 casos de sífilis en gestantes, con el mayor número de casos en el municipio de Macapá (646) y la mayor incidencia en Serra do Navio (26,55). La mayor parte de las notificaciones se produjeron en el edad de 20 a 29 años (47,7%), en el color marrón (74,75%), con educación primaria incompleta (22,21%), mujeres doméstico (58,82%), en el tercer trimestre gestacional (50,71%) y la clasificación de la enfermedad en fase primaria (27,28%). Para la confirmación diagnóstica, solo el 29,51% utilizó ambas pruebas, no treponémica y treponémica. El tratamiento predominó fue Penicilina G benzatínica 7.200.000 UI (49,39%). Los hallazgos indicaron un aumento anual en las notificaciones, así como un aumento considerable en el coeficiente de incidencia anual en las ciudades de Amapá, revelando la necesidad de interrumpir la cadena de transmisión de la sífilis.O estudo teve como objetivo descrever o perfil epidemiológico dos casos notificados de sífilis gestacional no Estado do Amapá no quinquênio de 2014 a 2018, associando com variáveis sociodemográficas e clínicas, e avaliar a distribuição espaço-temporal da doença nos municípios atráves de dados plotados em mapas usando o SIG. Estudo epidemiológico retrospectivo transversal, de natureza quantitativa e abordagem descritiva. As informações sobre as notificações de sífilis em gestantes foram retiradas do Sistema de Informação de Agravos de Notificação (SINAN), o número de nascidos vivos do Sistema de Informação sobre Nascidos Vivos (SINASC) e as malhas geográficas do Instituto Brasileiro de Geografia e Estatística (IBGE). De 2014 a 2018, registraram-se 986 casos de sífilis em gestantes, com o maior número de casos no município de Macapá (646) e a maior incidência em Serra do Navio (26,55). A maioria das notificações ocorreu na faixa etária de 20 a 29 anos (47,7%), na raça/cor parda (74,75%), em gestantes com 5ª a 8ª série incompleta (22,21%), em donas de casa (58,82%), no terceiro trimestre gestacional (50,71%) e a classificação da doença na fase primária (27,28%). Para confirmação diagnóstica, somente 29,51% utilizaram ambos os testes, não treponêmico e treponêmico. O tratamento que prevaleceu foi Penicilina G benzatina 7.200.000 UI (49,39%).
