Emergency surgery during the COVID-19 Pandemic

Resumo: Since March 11, 2020, the world has been experiencing a new coronavirus pandemic, caused by the disease COVID-19, which has resulted in several changes in daily life. Concerns about overloading the healthcare system and fear of contracting the virus may have led to late presentations and worse outcomes for patients with acute conditions. The objective of the study is to identify and report the effects of the pandemic on emergency surgery services. This is an integrative literature review, with a descriptive approach. The bibliographic research had as its guiding question "What are the impacts of the COVID-19 pandemic on surgical emergency services?". The databases used were: SciELO, PubMed and LILACS, based on the descriptors "COVID-19" and "Emergency Medical Services", using the Boolean operator AND. There were no epidemiologically significant changes in terms of gender and age. The number of presentations to urgent and emergency surgical services reduced by 21%. Complication rates increased by 48%, along with the mortality rate, which increased by 12%, and the late presentation rate to emergency services by 37.4%. The pandemic of the new SARS-CoV 2 significantly influenced the assistance of health systems. During this period, there was a reduction in these surgeries and in the number of acute events, such as trauma. Several hypotheses have been postulated to explain this behavior, including social isolation and information broadcast by the media about the situation in hospitals, which increases the delay in seeking emergency services and increases the mortality rate.

Las preocupaciones sobre la sobrecarga del sistema de atención médica y el miedo a contraer el virus pueden haber llevado a presentaciones tardías y peores resultados para los pacientes con afecciones agudas. El objetivo del estudio es identificar y reportar los efectos de la pandemia en los servicios de cirugía de emergencia. Se trata de una revisión integrativa de la literatura, con abordaje descriptivo. La investigación bibliográfica tuvo como pregunta orientadora "¿Cuáles son los impactos de la pandemia de COVID-19 en los servicios de emergencia quirúrgica?". Las bases de datos utilizadas fueron: SciELO, PubMed y LILACS, con base en los descriptores “COVID-19” y “Servicios Médicos de Emergencia”, utilizando el operador booleano AND. No hubo cambios significativos en relación con el sexo y la edad. El número de llamadas se redujo en un 21%. Las tasas de complicaciones aumentaron un 48%, junto con la tasa de mortalidad, que aumentó un 12%, y la tasa de presentación tardía a los servicios de urgencias un 37,4%. La nueva pandemia de SARS-CoV 2 ha influido significativamente en la asistencia de los sistemas de salud. Durante este período, hubo una reducción en estas cirugías y en el número de eventos agudos, como traumatismos. Se han postulado varias hipótesis para explicar este comportamiento, entre ellas el aislamiento social y la información difundida por los medios de comunicación sobre la situación en los hospitales, lo que aumenta la demora en la búsqueda de servicios de emergencia y aumenta la tasa de mortalidad.Desde 11 de março de 2020, o mundo vive uma nova pandemia do coronavírus, causada pela doença COVID-19, o que resultou em diversas mudanças no cotidiano. A preocupação de sobrecarregar o sistema de saúde e o medo de contrair o vírus podem ter levado a apresentações tardias e piores desfechos para pacientes com quadros agudos. O objetivo do estudo é identificar e relatar os efeitos da pandemia nos serviços de cirurgias de urgência. Trata-se de uma revisão integrativa da literatura, com abordagem descritiva. A pesquisa bibliográfica teve como questão norteadora “Quais os impactos da pandemia de COVID-19 nos serviços de emergência cirúrgica?”. As bases de dados utilizadas foram: SciELO, PubMed e LILACS, com base nos descritores "COVID-19" e "Serviços Médicos de Emergência", utilizando o operador booleano AND. Não houve alterações significativas em relação ao sexo e à idade. O número de atendimentos reduziu em 21%. A pandemia do novo SARS-CoV 2 influenciou significativamente a assistência dos sistemas de saúde. Nesse período, houve redução dessas cirurgias e do número de eventos agudos, como traumas.
