Evidence associated with the metabolic effect of carbohydrate restriction in patients with type II diabetes mellitus: An integrative literature review

Resumo: | Type 2 Diabetes Mellitus (DM2) is one of the non-communicable pathologies that have a high prevalence, incidence in Brazil and in the world, and may be related to the development of cardiovascular diseases and other comorbidities. Evidence suggests that dietary management based on carbohydrate restriction has significant effects on the course of the disease, given that it is a nutrient that has the greatest impact on postprandial blood glucose. The aim of the study was to highlight the metabolic effects that carbohydrate restriction provides in individuals with type II diabetes mellitus. This is an integrative literature review carried out in the Virtual Health Library (VHL) and PubMed. Controlled and randomized clinical trials published in the last 6 years in English were prioritized, among the exclusion criteria, all studies that did not approach the selected methodology in question. The following Health Sciences Descriptors (DeCS) were used: “carbohydrate-restricted diet”, “type 2 diabetes mellitus” and diet, for this purpose, the Boolean operator “AND” was used. The results indicate that a low-carbohydrate diet results in a decrease in cardiometabolic indicators, such as: better control of metabolic parameters related to glycemic and lipid profiles, greater predisposition to reduce anthropometric measurements and less need for hypoglycemic drugs. Therefore, it is concluded that the dietary approach is fundamental in the treatment and prevention of this disorder, especially when based on carbohydrate restriction. |

La evidencia sugiere que el manejo dietético basado en la restricción de carbohidratos tiene efectos significativos en el curso de la enfermedad, dado que es el nutriente que tiene mayor impacto en la glucemia posprandial. El objetivo del estudio fue resaltar los efectos metabólicos que proporciona la restricción de carbohidratos en individuos con diabetes mellitus tipo II. Se trata de una revisión integrativa de la literatura realizada en la Biblioteca Virtual en Salud (BVS) y PubMed. Los ensayos clínicos controlados y aleatorizados publicados en los últimos 6 años en idioma inglés se priorizaron, entre los criterios de exclusión, todos los estudios que no abordaran la metodología seleccionada en cuestión. Se utilizaron los siguientes Descriptores de Ciencias de la Salud (DeCS): “dieta restringida en carbohidratos”, “diabetes mellitus tipo 2” y dieta, para lo cual se utilizó el operador booleano “AND”. Los resultados indican que una dieta baja en carbohidratos resulta en una disminución de los indicadores cardiometabólicos, tales como: mejor control de los parámetros metabólicos relacionados con los perfiles glucémicos y lipídicos, mayor predisposición a reducir las medidas antropométricas y menor necesidad de fármacos hipoglucemiantes. Por lo tanto, se concluye que el abordaje dietético es fundamental en el tratamiento y prevención de este trastorno, especialmente cuando se basa en la restricción de carbohidratos.A Diabetes Mellitus tipo 2 (DM2) é uma das patologias não transmissíveis que possuem grande prevalência, incidência no Brasil e no mundo, podendo está relacionada com desenvolvimento de doenças cardiovasculares e outras comorbidades. Evidências sugerem que o manejo dietoterápico a partir da restrição de carboidratos apresenta efeitos significativos no curso da doença, tendo em vista que é um nutriente que apresenta maior impacto no glicemia pós-prandial. O objetivo do estudo foi evidenciar os efeitos metabólicos que a restrição de carboidratos proporciona em indivíduos com diabetes mellitus tipo II. Trata-se de uma revisão integrativa de literatura realizada na biblioteca Virtual em Saúde (BVS) e na PubMed. Primou-se por ensaios clínicos controlados e randomizados publicados nos últimos 6 anos no idioma inglês, dentre os critérios de exclusão, todos os estudos que não abordagem a metodologia selecionada em questão. Utilizou-se os seguintes Descritores em Ciências da Saúde (DeCS): “dieta com restrição de carboidrato”, “diabetes mellitus tipo 2” e dieta, para tanto, utilizou-se o operador booleano “AND”. Os resultados apontam que a dieta com baixo teor de carboidrato repercutem em queda dos indicadores cardiometabólicos, como: melhor controle nos parâmetros metabólicos relacionados a perfil glicêmico e lipídico, maior predisposição a redução das medidas antropométricas e menor necessidade do uso medicamentos hipoglicemiantes.
