Association between hormonal contraceptive methods and mental health problems

Resumo: | Introduction: Mental disorders encompass a set of psychiatric dysfunctions with negative repercussions on the biological body. In this context, studies suggest that women present twice the risk of depressive disorders when compared to men. Objective: To conduct a literature review in order to assess the association between the use of hormonal contraceptives and the development of mental health disorders. Methodology: Integrative literature review on the PubMed database of productions from the last 12 years. The descriptors used were: "Hormonal Contraceptive", "Psychiatric Disorder", "Mental Health" and "Women's Health", which were combined to delimit the search. Result: Seven articles were selected at the end of the search steps. The year 2016 was the most prevalent with 28.57% of the publications. Regarding the country of origin of the studies, 42.85% were conducted in the United States of America. It was found, from the Integrative Review, that the use of hormonal contraceptives attenuates symptoms of the menstrual cycle, such as anger and irritability. However, worsening of depression-related symptoms, and reduced sexual desire, arousal, and pleasure have also been reported. It is also seen that the combination with antidepressants may not be effective in treating premenstrual depression. There is also an association of worsening mood in women with a history of mood, anxiety and eating disorders. Conclusion: mental state changes were evidenced with the use of hormonal contraceptives, with emphasis on the worsening of depressive symptoms, sexual pleasure and anxiety, especially in those patients with a previous history of mood disorders. |

En este contexto, los estudios sugieren que las mujeres presentan el doble de riesgo de sufrir trastornos depresivos en comparación con los hombres. Objetivo: Realizar un análisis literario para evaluar la relación entre el uso de anticonceptivos hormonales y el desarrollo de afecciones a la salud mental. Metodología: Revisión integrativa de la literatura en base a los datos PubMed de las producciones de los últimos 12 años. Los descriptores utilizados fueron: "Anticonceptivo hormonal", "Trastorno psiquiátrico", "Salud mental" y "Salud de la mujer", que se combinaron para delimitar la búsqueda. Resultado: Al final de las etapas de búsqueda se seleccionaron siete artículos. Siendo el año 2016 el más prevalente con el 28,57% de las publicaciones. En cuanto al país de origen de los estudios, el 42,85% se realizaron en Estados Unidos de América. Según la revista Integrative Review, el uso de anticonceptivos hormonales atenúa los síntomas del ciclo menstrual, como la ira y la irritabilidad. Sin embargo, también se informó de un empeoramiento de los síntomas relacionados con la depresión, así como de una reducción del deseo, la excitación y el placer sexuales. También se observa que la combinación con antidepresivos puede no ser eficaz en el tratamiento de la depresión premenstrual. También existe una asociación de empeoramiento del estado de ánimo en mujeres con antecedentes de trastornos del estado de ánimo, ansiedad y alimentación. Conclusión: se evidenciaron alteraciones del estado mental con el uso de anticonceptivos hormonales, con destaque para el aumento de los síntomas depresivos, de la prazer sexual y de la ansiedad, especialmente en los pacientes con historia previa de alteraciones del humor.Introdução: As desordens mentais abrangem um conjunto de disfunções psiquiátricas com repercussões negativas ao corpo biológico. Nesse contexto, estudos sugerem que as mulheres apresentam o dobro do risco de transtornos depressivos quando comparadas aos homens. Objetivo: Realizar uma análise literária a fim de avaliar a associação entre o uso de anticoncepcionais hormonais e o desenvolvimento de afecções à saúde mental. Metodologia: Revisão Integrativa da literatura na base de dados PubMed das produções dos últimos 12 anos. Os descritores utilizados foram: “Hormonal Contraceptive”, “Psychiatric Disorder”, “Mental Health” e “Women’s Health”, os quais foram conjugados para delimitação da busca. Resultado: Foram selecionados 7 artigos ao final das etapas de busca. Sendo o ano de 2016 o mais prevalente com 28,57% das publicações. Em relação ao país de origem dos estudos, 42,85% foram conduzidos nos Estados Unidas da América. Contatou-se, a partir da Revisão Integrativa, que uso de contraceptivo hormonal atenua sintomas do ciclo menstrual, como raiva e irritabilidade. Porém, também foi relatado piora de sintomas relacionado à depressão, além de redução de desejo, excitação e prazer sexual. Vê-se também que a combinação com antidepressivos pode não ser eficaz no tratamento da depressão pré-menstrual. Também há associação de piora de humor em mulheres com história de desordens de humor, ansiedade e alimentação.
