Pain assessment and management in a neonatal intensive care unit: The perspective of the nursing team

Resumo: This study aimed to identify the knowledge and practices of the nursing team regarding the assessment and management of pain in newborns admitted to the Neonatal Intensive Care Unit. This is an exploratory-descriptive research with a qualitative approach, whose final sample included 21 subjects. Data were collected through a semi-structured questionnaire from the google forms platform. With regard to the evaluation of the painful process, the evaluation scales used by the team include the NIPS and COMFORT-B. As for the management of neonatal pain, it was mentioned that analgesic therapies are used only in specific procedures after assessing the real need, such as in cases of phlebotomy, orotracheal intubation, chest drain and passage of Peripherally Inserted Central Catheter (PICC). With regard to non-pharmacological measures, the professionals mentioned the routine use of measures such as the kangaroo method/skin-to-skin contact, positioning, the cigar technique and non-nutritive sucking. The nursing team demonstrated that they considered the importance of controlling and treating this vital sign and its possible short- and long-term repercussions when neglected. It was evident that some professionals do not use and do not perceive a favorable impact on the use of scales, claiming that only the subjective evaluation of the team is sufficient.

Se trata de una investigación exploratoria-descriptiva con enfoque cualitativo, cuya muestra final estuvo compuesta por 21 sujetos. Los datos fueron recolectados a través de un cuestionario semiestructurado de la plataforma de google forms. En cuanto a la evaluación del proceso doloroso, las escalas de evaluación utilizadas por el equipo incluyen la NIPS y la COMFORT-B. En cuanto al manejo del dolor neonatal, se mencionó que las terapias analgésicas se utilizan solo en procedimientos específicos después de evaluar la necesidad real, como en los casos de flebotomía, intubación orotraqueal, drenaje torácico y paso de Catéter Central de Inserción Periférica (PICC). Con relación a las medidas no farmacológicas, los profesionales mencionaron el uso rutinario de medidas como el método canguro/contacto piel con piel, posicionamiento, técnica del cigarro y succión no nutritiva. El equipo de enfermería demostró considerar la importancia del control y tratamiento de esta constante vital y sus posibles repercusiones a corto y largo plazo cuando se descuida. Se evidenció que algunos profesionales no utilizan y no perciben un impacto favorable en el uso de las escalas, alegando que solo la evaluación subjetiva del equipo es suficiente.Este estudo objetivou identificar o conhecimento e as práticas da equipe de enfermagem acerca da avaliação e do manejo da dor de recém-nascidos admitidos em Unidade de Terapia Intensiva Neonatal. Trata-se de uma pesquisa exploratório-descritiva com abordagem qualitativa, cuja amostra final incluiu 21 sujeitos. Os dados foram coletados por meio de um questionário semiestruturado a partir da plataforma google forms. De acordo com os resultados, as alterações mais apontadas frente à dor foram a expressão facial, agitação e choro, além da mudança dos sinais vitais. No que diz respeito à avaliação do processo doloroso, as escalas de avaliação utilizadas pela equipe incluem a NIPS e COMFORT-B. Quanto ao manejo da dor neonatal, referiu-se que as terapias analgésicas são utilizadas apenas em procedimentos específicos após avaliação da real necessidade, como nos casos de flebotomia, intubação orotraqueal, dreno de tórax e passagem de Cateter Central de Inserção Periférica (PICC). Já no que tange às medidas não farmacológicas, os profissionais citaram como rotina o emprego de medidas como o método canguru/contato pele a pele, posicionamento, técnica do charutinho e sucção não nutritiva. A equipe de enfermagem demonstrou considerar a importância do controle e tratamento deste sinal vital e suas possíveis repercussões a curto e longo prazo quando negligenciado.
