The quality of prenatal care in primary health care in Brazil: an integrative literature review

Resumo: The assessment of the quality of prenatal health care is related to a potential impact on prematurity rates and on the reduction of infant morbidity and mortality rates in the country. Objectives: to relate the results found based on low-risk prenatal policies in primary care; identify, based on the literature, what is being discussed on the topic; building a demonstrative table with the factors involved in the quality of service in low-risk prenatal care found in the literature. Methodology: this is an integrative review of the literature in the light of the theoretical Donabedian. No time frame was included in the search in order not to reduce likely useful content for the search. The databases consulted were: Web of Science, VHL and VHL Public Health, from November 2020 to March 2021 and a survey was made based on the respective queries. Results: the vast majority of articles address criteria that are consistent with professional practice and service protocols. While it is possible to observe that few crossings were made between the three elements proposed by Donabedian, which are structure, process and results. Conclusion:  from this study it is possible to understand about the need to expand studies on the subject, mainly in cross-sectional studies, which present proposals for improving the quality of care. Enabling new discussions, in addition to observing the findings from a more complete perspective, since they directly affect the health care that is offered in the different perinatal scenery.

Objetivos: relacionar los resultados encontrados a partir de las políticas prenatales de bajo riesgo en la atención primaria; identificar con base en la literatura lo que se está discutiendo sobre el tema; construir un cuadro demostrativo con los factores involucrados en la calidad de la atención prenatal de bajo riesgo encontrados en la literatura. Metodología: se trata de una revisión integrativa de la literatura a la luz del teórico Donabedian. No se incluyó ningún período de tiempo en la búsqueda para no reducir el contenido probablemente útil para la búsqueda. Las bases de datos consultadas fueron: Web of Science, BVS y BVS Salud Pública, de noviembre de 2020 a marzo de 2021 y se realizó un relevamiento a partir de las respectivas consultas. Resultados: en su mayoría, los artículos abordan criterios que son consistentes con la práctica profesional y los protocolos de atención. Si bien es posible observar que se realizaron pocos cruces entre los tres elementos propuestos por Donabedian, que son estructura, proceso y resultados. Conclusión: a partir de este estudio es posible comprender la necesidad de ampliar los estudios sobre el tema, especialmente en estudios transversales, que presenten propuestas para mejorar la calidad de la atención. Permitiendo profundizar en las discusiones, además de observar los hallazgos desde una perspectiva más completa, ya que ejercen efectos directos en la atención a la salud que se ofrece en los diversos escenarios perinatales.A avaliação da qualidade do serviço de saúde pré-natal está relacionada a um impacto potencial nas taxas de prematuridade e na redução das taxas de morbimortalidade infantil no país. Objetivos: relacionar os resultados encontrados pautados nas políticas de pré-natal de baixo risco na atenção básica; identificar baseado na literatura o que está sendo discutido acerca do tema; construir quadro demonstrativo com os fatores intervêm na qualidade do serviço no pré-natal de baixo risco encontrados na literatura. Metodologia: trata-se de uma revisão integrativa da literatura à luz do teórico Donabedian. Não foi incluído recorte temporal na pesquisa a fim de não reduzir prováveis conteúdos úteis para a busca. As bases de dados consultadas foram: Web of Science, BVS e BVS Saúde Pública, no período novembro de 2020 a março de 2021 e foi feito levantamento a partir dos respectivos queries. Resultados: em sua grande maioria, os artigos abordam critérios que condizem com a prática profissional e protocolos de atendimento. Enquanto é possível observar que foram feitos poucos atravessamentos entre os três elementos propostos por Donabedian, que são estrutura, processo e resultados. Conclusão: a partir desse estudo é possível compreender a respeito da necessidade de ampliação dos estudos na temática, principalmente em estudos transversais, que apresentem propostas de melhoria da qualidade do atendimento.
