Elaboration of an Instrument to Systematize Nursing Care in a Vascular Cerebral Accident Unit

Resumo: Stroke is a disabling disease, with high mortality, requiring rapid identification of signs and symptoms for immediate intervention. This study aimed to report the experience about the application of the Nursing Process (NP) in a Stroke Unit. It is an experience report, with a qualitative approach, about the nurses' experience of a Stroke Unit in view of the application of NP. The NP presented a diagnostic instrument and a nursing intervention instrument based on the main changes that the patient diagnosed with a stroke can present.  However, it continued without updates since 2009. All the instruments constructed containing the diagnoses, results and nursing interventions related to the stroke patient were gathered and kept in a single document folder at the U-Stroke for consultation by nurses during the shift. The NP was reformulated, including expected results, new activities and design, providing better acceptance and applicability by the nursing team. The adoption of new instruments for the operationalization of the NP generated a demand for activities, improving the assistance and care provided to the patient.

Este estudio tuvo como objetivo informar la experiencia sobre la aplicación del Proceso de Enfermería (PE) en una Unidad de Accidente Vascular Cerebral. Es un informe de experiencia, con un enfoque cualitativo, sobre la experiencia de las enfermeras en un Unidad de Accidente Vascular Cerebral en vista de la aplicación de PE en esa unidad. El PE presentó un instrumento de diagnóstico y un instrumento de intervención de enfermería basado en los principales cambios que el paciente diagnosticado con un accidente cerebrovascular puede presentar. Sin embargo, continuó sin actualizaciones desde 2009. Todos los instrumentos construidos que contienen los diagnósticos, resultados e intervenciones de enfermería relacionadas con el paciente con ictus fueron recolectados y guardados en una sola carpeta de documentos en el U-Stroke para la consulta de las enfermeras durante el turno. El PE fue reformulado, incluyendo los resultados esperados, nuevas actividades y diseño, proporcionando una mejor aceptación y aplicabilidad por el equipo de enfermería. La adopción de nuevos instrumentos para la operacionalización del PN, generó una demanda de actividades, mejorando la asistencia y la atención prestada al paciente.O AVC é uma doença incapacitante, com elevada mortalidade, necessitando de identificação rápida dos sinais e sintomas para intervenção imediata. Este estudo teve por objetivo relatar a experiência acerca da aplicação do Processo de Enfermagem (PE) em uma Unidade de Acidente Vascular Cerebral (U-AVC). Trata-se de um relato de experiência, de abordagem qualitativa, acerca da vivência de enfermeiros de uma U-AVC face à aplicação do PE na referida unidade. O PE apresentava um instrumento de diagnósticos e outro de intervenções de enfermagem baseada nas principais alterações que o paciente diagnosticado com AVC pode apresentar. Entretanto, seguia sem avaliações/atualizações desde 2009. Foram reunidos todos os instrumentos construídos contendo os diagnósticos, resultados e intervenções de enfermagem relacionadas ao paciente com AVC e mantidos em uma única pasta-documento na U-AVC para consulta pelos enfermeiros durante o plantão.  O PE foi reformulado, incluindo resultados esperados, novas atividades e design, proporcionando melhor aceitação e aplicabilidade pela equipe de enfermagem.
