Porphyromonas gingivalis as a link between periodontitis and Alzheimer's disease. Narrative literature review

Resumo: Alzheimer's disease (AD) is one of the most common neurodegenerative disorders in the world and has been listed as a global public health priority by the WHO. There is currently no effective treatment for the disease and its cause is still unknown. In recent years, attempts have been made to explain the etiological mechanisms of AD, one proposed hypothesis is the spread of oral microorganisms such as Porphyromonas gingivalis (Pg) to brain tissue in patients with periodontitis. However, this possible causal relationship remains unclear. Objective: to present the role of Pg as a link between periodontitis and Alzheimer's disease based on a literature review of publications from the last 5 years. Materials and methods: search of publications in the following databases: PubMed, SpringerLink, Scientific Direct, Scielo. Results: 19 scientific articles were included of which animal and molecular studies determine that, Pg infection increased the production of inflammatory mediators, thus causing an increase in the production of Aβ (beta amyloid) and subsequently, neurodegeneration compatible with AD; however, no clear relationship has been established in human studies. Conclusion: the effect of Pg on the development of neuroinflammation has been demonstrated; however, further large-scale studies and longitudinal follow-up in humans are needed to better establish the relationship between periodontitis and AD.

Actualmente no existe un tratamiento efectivo para la enfermedad y su causa aún se desconoce. En los últimos años, se ha tratado de explicar los mecanismos etiológicos de la EA y se ha propuesto la hipótesis de la propagación de microorganismos orales como la Porphyromonas gingivalis (Pg) hacia el tejido cerebral en pacientes con periodontitis. Sin embargo, esta posible relación causal sigue sin estar clara. Objetivo: presentar el papel de la Pg como vínculo entre la periodontitis y la enfermedad de Alzheimer a partir de una revisión de literatura de publicaciones de los últimos 5 años. Materiales y métodos: búsqueda de publicaciones en las bases de datos: PubMed, SpringerLink, Scientific Direct, Scielo. Resultados: se incluyeron 19 artículos científicos de los cuales los estudios en animales y moleculares determinan que, la infección por Pg aumentó la producción de los mediadores inflamatorios, provocando así un aumento en la producción de Aβ (beta amiloide) y posteriormente, neurodegeneración compatible con la EA; sin embargo, no se ha establecido una relación clara en los estudios realizados en humanos. Conclusión: se ha demostrado el efecto de la Pg en el desarrollo de neuroinflamación; sin embargo, se necesitan estudios adicionales a gran escala y seguimiento longitudinal en humanos para establecer de mejor manera la relación entre la periodontitis y la EA.A doença de Alzheimer (DA) é um dos distúrbios neurodegenerativos mais comuns no mundo e foi listada como uma prioridade de saúde pública global pela OMS. Entretanto, essa possível relação causal ainda não está clara. Objetivo: apresentar o papel da Pg como um elo entre a periodontite e a doença de Alzheimer com base em uma revisão da literatura de publicações dos últimos 5 anos. Materiais e métodos: pesquisa de publicações nos seguintes bancos de dados: PubMed, SpringerLink, Scientific Direct, Scielo. Resultados: foram incluídos 19 artigos científicos dos quais os estudos em animais e moleculares determinaram que a infecção por Pg aumentava a produção de mediadores inflamatórios, causando assim, um aumento na produção de Aβ (beta amiloide) e, subsequentemente, neurodegeneração compatível com a DA; no entanto, nenhuma relação clara foi estabelecida em estudos em humanos.
