Characteristics of refugee women's access to health services during pregnancy and puerperium: an integrative review

Resumo: | Objective: To characterize the access to health services of refugee women in their pregnancy-puerperal cycle in the global context. Methods: Integrative literature review whose searches were performed in PUBMED, LILACS and CINAHL databases. Studies published from 1996 to 2021 were observed. Results: Sixteen studies were selected for the final sample, according to the previously established criteria. Two themes were highlighted: Characteristics of access to health services by refugee women in the pregnancy-puerperal period and strategies used to promote better access to health. The analysis and synthesis of the results revealed the following characteristics: language barriers, late search for care, difficulties in obtaining health insurance, higher incidence of complications in pregnancy and puerperium, higher incidence of depression, problems with means of transportation, stigma and cultural discrimination, legal difficulties related to immigration, among others, as well as successful strategies and specific recommendations. Conclusion: By characterizing access, the study revealed barriers encountered by refugees in obtaining access to health care. Thus, it is essential to establish public policies worldwide that can provide adequate access to health care during pregnancy and puerperium to this vulnerable population. |

Métodos: Revisión bibliográfica integradora cuyas búsquedas se realizaron en las bases de datos PUBMED, LILACS y CINAHL. Se observaron los estudios publicados entre 1996 y 2021. Resultados: Se seleccionaron dieciséis estudios para la muestra final según los criterios previamente establecidos. Se evidenciaron dos temas: Características del acceso a los servicios sanitarios por parte de las mujeres refugiadas en el periodo gravídico-puerperal y estrategias utilizadas para promover un mejor acceso a la salud. Los análisis y las síntesis de los participantes revelaron las siguientes características: barreras idiomáticas, tardanza en la atención, dificultades para obtener un seguro de salud, mayor incidencia de complicaciones en la gestación y el parto, mayor incidencia de depresión, problemas con los medios de locomoción, estigma y discriminación cultural, dificultades legales relacionadas con la inmigración, entre otras, además de estrategias de éxito ponderadas y recomendaciones. Conclusión: Al caracterizar el acceso, el estudio reveló las barreras encontradas por las refugiadas para obtener acceso a la salud. De este modo, es esencial el establecimiento de políticas públicas en el ámbito mundial que permitan ofrecer un acceso adecuado a la asistencia sanitaria durante el embarazo y el parto a esta población vulnerable.Objetivo: Caracterizar o acesso aos serviços de saúde de mulheres refugiadas em seu ciclo gravídico-puerperal no contexto mundial. Métodos: Revisão integrativa da literatura cujas buscas foram conduzidas nas bases de dados PUBMED, LILACS e CINAHL. Observaram-se estudos publicados a partir de 1996 a 2021. Resultados: Foram selecionados 16 estudos para compor a amostra final conforme os critérios previamente estabelecidos. Evidenciaram-se duas temáticas: Características do acesso aos serviços de saúde por mulheres refugiadas no período gravídico-puerperal e estratégias utilizadas para promover melhor acesso à saúde. As análises e sínteses dos achados revelaram as seguintes características: barreiras idiomáticas, busca tardia por atendimento, dificuldades em obter seguro saúde, maior incidência de complicações na gestação e puerpério, maior incidência de depressão, problemas com meios de locomoção, estigma e discriminação cultural, dificuldades legais relacionadas à imigração entre outros, além de estratégias de sucesso pontuais e recomendações. Conclusão: Ao caracterizar o acesso, o estudo revelou barreiras encontradas pelas refugiadas em obter acesso à saúde.
