Physical disabilities of hanseníasis in children under 15 in the state of Tocantins, Brazil, 2001 to 2020

Resumo: To describe the trend of physical disabilities caused by leprosy in children in the state of Tocantins. An analysis of the calculations of leprosy indicators and temporal trends was carried out in the period from 2001 to 2020. 1,983 new cases of leprosy were diagnosed in children under 15 years of age. Degree 2 of physical disability was more prevalent in males (66.67%). The prevalence of cases with grade 1 and 2 of physical disability was higher in the multibacillary classification, but the paucibacillary classification had a record of 31.56% of grade 1 and 17.95% of grade 2. The detection coefficient and the proportion of cases with grade 2 showed stability in the trend in the total period. Between 2014 and 2020, the proportion of paucibacillary cases had a significant reduction of 26.2%, while in the period 2014 and 2018, multibacillary cases had an increase of 38.2%. The proportion of grade 1 cases had a significant increase of 16.3% between 2001 and 2010 and of 58.7% between 2014 and 2018 and stability in the total period. Physical disabilities in children and adolescents remain an important public health problem at the time of leprosy diagnosis in the state of Tocantins. The trend towards stability in disability points to the persistence of endemic disease, late diagnosis, hidden prevalence and sequelae in children. Operational failures and failures in the clinical management of health services were clearly identified, which reaffirms the need to make more efforts to improve leprosy care.

Se realizó un análisis de los cálculos de indicadores de lepra y tendencias temporales en el período 2001 a 2020. Se diagnosticaron 1.983 nuevos casos de lepra en menores de 15 años. El grado 2 de discapacidad física fue más prevalente en el sexo masculino (66,67%). La prevalencia de casos con grado 1 y 2 de discapacidad física fue mayor en la clasificación multibacilar, pero la clasificación paucibacilar tuvo un registro de 31,56% de grado 1 y 17,95% de grado 2. El coeficiente de detección y la proporción de casos con grado 2 mostró estabilidad en la tendencia en el total del período. Entre 2014 y 2020, la proporción de casos paucibacilares tuvo una reducción significativa del 26,2 %, mientras que en el período 2014 y 2018 los casos multibacilares tuvieron un aumento del 38,2 %. La proporción de casos grado 1 tuvo un aumento significativo de 16,3% entre 2001 y 2010 y de 58,7% entre 2014 y 2018 y estabilidad en el período total. Las discapacidades físicas en niños y adolescentes continúan siendo un importante problema de salud pública al momento del diagnóstico de lepra en el estado de Tocantins. La tendencia hacia la estabilidad en la discapacidad apunta a la persistencia de la enfermedad endémica, diagnóstico tardío, prevalencia oculta y secuelas en los niños. Se identificaron claramente fallas operativas y de gestión clínica, lo que reafirma la necesidad de realizar mayores esfuerzos para mejorar la atención de la lepra.Descrever a tendência de incapacidades físicas da hanseníase em crianças no estado do Tocantins. Entre os anos de 2014 e 2020, a proporção de casos paucibacilares tiveram uma redução significativa de 26,2%, enquanto no período de 2014 e 2018 os casos de multibacilares tiveram um aumento de 38,2%. A proporção de casos de grau 1 teve incremento significativo de 16,3% entre 2001 e 2010 e de 58,7% entre 2014 e 2018 e de estabilidade no período total. As incapacidades físicas em crianças e adolescentes mantêm-se como um importante problema de saúde pública no momento do diagnóstico da hanseníase no estado do Tocantins. A tendência de estabilidade nas incapacidades aponta a constância da endemia, diagnóstico tardio, prevalência oculta e sequelas em crianças.
