Breastfeeding in covid-19 times: an integrative review

Resumo: Objective: to realize a bibliographic survey in search of the association between the act of breastfeeding and the possibility of the vertical transmission of the SARS-CoV-2. Methodology: This is an integrative literature review and its leading question is “Is the breast milk able to transmit the SARS-CoV-2?”. In this way, in the period of 2020 June, indexed scientific publications were searched. Finally, 8 articles were included for satisfactorily respond to the leading question and, therefore, they composed the final group of this review analysis. Results: 37,5% of the studies reported no evidence of the presence of the SARS-CoV-2 in breast milk samples; 12,5% described about the non-transmissibility of the virus from infected mothers to their newborns through breastfeeding; 12,5% demonstrated indifference between the COVID-19 infection rate in children, who were breastfed or not; and, lastly, 37,5% revealed, in total, the presence of the SARS-CoV-2  in three breast milk samples from two patients Conclusions: Based on the findings, it was possible to observe that there is not any actual evidences in literature that proves the vertical transmission of the SARS-CoV-2 to newborns through the breastfeeding yet. As stated in the main current recommendations, the benefits of the breast milk overcome the possible risks of contamination, thus, the breastfeeding must be encouraged in COVID-19 infected mothers, since they adopt respiratory and personal adequate hygiene measures.

Metodología: Este es un estudio integrador de revisión de literatura y su pregunta orientadora es: “¿Es la leche materna capaz de transmitir el SARS-CoV-2?”. En ese sentido, en el período de junio de 2020, se buscaron publicaciones científicas indexadas. Finalmente, se incluyeron 8 artículos por responder satisfactoriamente a la pregunta guía y, luego, compusieron el conjunto final de análisis para esta revisión. Resultados: 37,7% de los estudios reportaron la ausencia de evidencias sobre la presencia de SARS-CoV-2 en muestras de leche materna; 12,5% han informado sobre la no transmisibilidad del virus de madres infectadas a sus recién nacidos amamantados; 12,5% han demostrado indiferencia entre las tasas de infección por COVID-19 en niños, amamantados o no; y, por fin, 37,5% han informado, en total, la presencia de SARS-CoV-2 en tres muestras de leche materna de dos pacientes. Conclusiones: en base a los hallazgos, fue posible observar que aún no existe evidencias consistentes en la literatura actual que prueban la transmisión vertical del virus SARS-CoV-2 a través del amamantamiento. Según las principales recomendaciones vigentes, se percibe que los beneficios de la leche materna superan posibles riesgos de contaminación, por lo tanto, el amamantamiento debe ser fomentado en madres infectadas con COVID-19, mientras sean tomadas medidas apropiadas para higiene respiratoria y personal.Objetivo: Realizar um levantamento bibliográfico em busca da associação entre a prática do aleitamento materno e a possibilidade de transmissão vertical do SARS-CoV-2. Metodologia: Trata-se de um estudo de revisão de literatura, do tipo integrativa e, como questão norteadora do estudo, definiu-se: “O leite humano é capaz de transmitir o SARS-CoV-2?”. Nesse sentido, no período de junho do ano de 2020, publicações científicas indexadas foram pesquisadas. Finalmente, 8 artigos foram incluídos por responderem satisfatoriamente à questão norteadora, e, portanto, compuseram o conjunto final de análise desta revisão. Resultados: 37,5% dos estudos relataram a ausência de evidências quanto a presença do SARS-COV-2 em amostras de leite materno; 12,5%  reportaram sobre a não transmissibilidade do vírus de mães infectadas para seus recém-nascidos sob aleitamento materno; 12,5% demonstraram indiferença entre as taxas de infecção por COVID-19 em crianças amamentadas ou não; e por fim, 37,5%  relataram, ao total, a presença do SARS-COV-2 em três amostras de leite materno colhidas em duas pacientes. Conclusões: Com base nos achados, pôde-se observar que ainda não há na literatura atual evidências consistentes que sustentem a transmissão vertical do vírus SARS-CoV-2 no período neonatal através da amamentação.
