The influence of Religiosity/Spirituality on the therapeutic response and prognosis of patients with mental disorders: an integrative review

Resumo: Religiosity/Spirituality (R/S) have become the object of interest by many health researchers, allowing care to become more compassionate and suitable to the patient’s integrality. The aims of this study were to analyze and describe the influence of R/S on the therapy and prognosis of patients with mental disorders. This is an integrative literature review based on the following databases: PubMed, BIREME, SciELO, Cochrane Library and Portal de Periódicos CAPES. Controlled descriptors combined with boolean operators were used: “mental disorders” AND “spirituality” AND “prognosis”; and, in the last databases: “spirituality” AND “mental disorders prognosis”. The data collection took place in April 2020. 753 articles were recovered 69 studies were included that filled on the inclusion criterias and describes the influence of R/S on the therapy and prognosis of Dementia, Affective/Mood Disorders, Personality Disorders, Schizophrenia, Post-Traumatic Stress Disorder, Alcohol and other drugs disorders, and mental health in general. The articles evidenced a decrease in hospitalization and in patient’s symptoms, in addition to an increase in treatment adherence, improvement in quality of life, lower rates of alcohol abuse and development of gratitude, optimism, purpose and generosity. Studies converge on spirituality as a protective factor in several psychiatric diseases, also acting as an ally to treatment and improving prognosis. Furthermore, there is also a cost reduction in hospitalizations, which are proven to be shorter when spirituality is considered in the care process.

Los objetivos fueron analizar y describir la influencia de la R/E en la terapia y pronóstico de pacientes con trastornos mentales. Es una revisión integradora de la literatura que utiliza las bases de datos PubMed, BIREME, SciELO, Cochrane Library y Portal de Periódicos CAPES. Se utilizaron descriptores controlados combinados con operadores booleanos: “mental disorders” AND “spirituality” AND “prognosis”; y, en la última: “spirituality” AND “mental disorders prognosis”. La recopilación de datos tuvo lugar en abril de 2020. Se recuperaron 753 artículos y se seleccionaron 69 que cumplieron con los criterios de inclusión y describen la influencia de la R/E en la terapia y pronóstico de la Demencia, Trastornos Afectivos/del Estado de Ánimo, de la Personalidad, Esquizofrenia, Estrés Postraumático, adicción al alcohol y otras drogas y salud mental, en general. Hay indicios de una reducción de la hospitalización y una disminución de los síntomas, además de una mayor adherencia al tratamiento, mejor calidad de vida, menores tasas de abuso de alcohol y mayor sentimiento de gratitud, optimismo, propósito y generosidad. Los estudios convergen en la espiritualidad como factor protector en varias enfermedades psiquiátricas, actuando como aliada del tratamiento y mejorando el pronóstico. También hay una reducción de los costos en las hospitalizaciones, que se comprueba que son más breves cuando la espiritualidad se inserta en la atención.Religiosidade/Espiritualidade (R/E) têm se tornado objeto de interesse de muitos estudiosos da saúde, permitindo que o cuidado se torne mais compassivo e adequado à integralidade dos pacientes. Os objetivos foram analisar e descrever a influência da R/E na terapêutica e prognóstico de pacientes com transtornos mentais. Trata-se de uma revisão integrativa da literatura utilizando as bases de dados PubMed, BIREME, SciELO, Cochrane Library e Portal de Periódicos CAPES. Foram utilizados os descritores controlados combinados com operadores booleanos: “mental disorders” AND “spirituality” AND “prognosis”; e, na última: “spirituality” AND “mental disorders prognosis”. A coleta de dados se deu em abril de 2020. Foram recuperados 753 artigos e 69 foram selecionados que atendem aos critérios de inclusão e descrevem a influência da R/E na terapêutica e prognóstico dos Transtornos Demenciais, Afetivos/de Humor, de Personalidade, da Esquizofrenia, do Estresse Pós-Traumático, da dependência do álcool e outras drogas e da saúde mental, em geral. Percebe-se o indicativo da redução do tempo de internação e declínio da sintomatologia, além de aumento da adesão ao tratamento, melhora da qualidade de vida, menores taxas de abuso de álcool e aumento no sentimento de gratidão, otimismo, propósito e generosidade. Os estudos convergem para a espiritualidade como fator protetor em diversas doenças psiquiátricas, atuando também como aliada ao tratamento e melhorando o prognóstico.
