The importance of pharmaceutical guidance at hospital discharge and in the post-discharge self-care process: an integrative review

Resumo: | This research aims to address pharmaceutical guidance at hospital discharge since in the discharge process, in the transition from hospitalization to self-care, where the patient is vulnerable, medication reconciliation and effective communication between the team and the patient are extremely important. Methods: An integrative literature review carried out in the Virtual Health Library (BVS) and Pubmed databases, using the domains “patient safety”, “hospital discharge” and “medication reconciliation”. Publications were selected according to inclusion and exclusion criteria; the analysis of the articles was done by pairs; Languages: English and Portuguese. The search in the databases resulted in 13 studies. For data processing and analysis, the IRAMUTEC software and the application of the URSI form were used. Results: The present RIL has a good level of scientific evidence, helping to expand knowledge on the subject. The Descending Hierarchical Classification (CHD) identified three semantic classes according to the analysis of textual domains and interpretation of meanings: 1- Changes and therapeutic changes as the main cause of damage and adverse events after hospital discharge; 2- Health education and medication guidance as a stimulus to self-care after hospital discharge; 3- Information and home monitoring to improve the problem regarding the use of medications after hospital discharge. Conclusion: The present review made it possible to clarify the importance of the pharmacist in the discharge orientation and how he can intervene in the self-care process, even though his role is not well defined. |

Métodos: Revisión integrativa de la literatura realizada en la Biblioteca Virtual en Salud (BVS) y en las bases de datos Pubmed, utilizando los dominios “seguridad del paciente”, “alta hospitalaria” y “conciliación de medicamentos”. Las publicaciones fueron seleccionadas según criterios de inclusión y exclusión; el análisis de los artículos se hizo por parejas; Idiomas: inglés y portugués. La búsqueda en las bases de datos resultó en 13 estudios, para el procesamiento y análisis de datos se utilizó el software IRAMUTEC y la aplicación del formulario URSI. Resultados: La presente EIR tiene un buen nivel de evidencia científica, ayudando a ampliar el conocimiento sobre el tema. La Clasificación Jerárquica Descendente (CHD) identificó tres clases semánticas según el análisis de dominios textuales e interpretación de significados: 1- Alteraciones y cambios terapéuticos como principal causa de daño y eventos adversos después del alta hospitalaria; 2- Educación en salud y orientación sobre medicamentos como estímulo para el autocuidado después del alta hospitalaria; 3- Información y seguimiento domiciliario para mejorar la problemática del uso de medicamentos tras el alta hospitalaria. Conclusión: La presente revisión permitió esclarecer la importancia del farmacéutico en la orientación al alta y cómo puede intervenir en el proceso de autocuidado, aunque su rol no esté bien definido.Essa pesquisa visa abordar a orientação farmacêutica na alta hospitalar visto que no processo de alta, na transição da internação para o autocuidado, onde o paciente fica vulnerável é de extrema importância a reconciliação medicamentosa e uma comunicação eficaz entre a equipe e o paciente. Métodos: Revisão integrativa da literatura realizada nas bases de dados Biblioteca Virtual de Saúde (BVS) e Pubmed, utilizando se os domínios “segurança do paciente”, “alta hospitalar” e “reconciliação medicamentosa”. As publicações foram selecionadas de acordo com os critérios de inclusão e exclusão; a análise dos artigos se deu por pares; idiomas: inglês e português. A busca nas bases de dados resultou em 13 estudos, para o processamento e análise dos dados, utilizou-se o software IRAMUTEC e a aplicação do formulário URSI. Resultados: A presente RIL tem um bom nível de evidência científica, colaborando para ampliar os conhecimentos acerca da temática. A Classificação Hierárquica Descendente (CHD) identificou três classes semânticas conforme a análise dos domínios textuais e interpretação dos significados: 1- Alterações e trocas terapêuticas como principal causa de danos e eventos adversos após a alta hospitalar; 2- Educação em saúde e orientação medicamentosa como estímulo ao autocuidado após a alta hospitalar; 3- Informação e acompanhamento domiciliar para a melhora do problema em relação a uso de medicamentos após a alta hospitalar.
