Evaluation of Pharmaceutical Interventions: study carried out in a Pediatric ICU of the University Hospital Materno Infantil in São Luís - MA

Resumo: | Patient safety has been a topic of discussion in health institutions, especially in Pediatric Intensive Care Units, since errors and their consequences are more serious in intensive care patients. Pharmaceutical intervention is part of pharmacotherapeutic monitoring and requires record, carried out jointly between health professionals and the patient, with the purpose of preventing and solving negative clinical results. Thus, the aim of this work is to evaluate the profile of pharmaceutical interventions performed by clinical pharmacy in pediatric patients admitted to the Pediatric Intensive Care Unit (PICU) of the Materno Infantil University Hospital in São Luís - MA. This is a quantitative, retrospective and cross-sectional study, based on a survey of data provided by the clinical pharmacy carried out through the analysis of the medical records of pediatric patients, from which information on pharmaceutical interventions recorded was taken in the database from January 2017 to December 2018. A total of 1003 interventions were obtained through participation in multidisciplinary visits at the PICU by pharmacists from the clinical pharmacy team. Of these 1003 pharmaceutical interventions, 98% (n= 983) were accepted and found to have a higher prevalence, in 2017, dose/dosage adjustment 51% and the most suitable therapeutic alternative available 25%. Additionally, in the 2018 period, there was a 37% drop in dose/dosing adjustment. It is concluded that pharmaceutical interventions contributed to the reduction of errors related to the pharmacotherapy of these children and also to the improvement of their clinical condition, a cutback in treatment costs and in the hospitalization period. |

La intervención farmacéutica forma parte del seguimiento farmacoterapéutico y requiere un registro, realizado de forma conjunta entre los profesionales sanitarios y el paciente, con el objetivo de prevenir y solucionar los resultados clínicos negativos. Así, el objetivo de este estudio fue evaluar el perfil de las intervenciones farmacéuticas realizadas por la farmacia clínica en pacientes pediátricos ingresados en la Unidad de Cuidados Intensivos Pediátricos (UTIP) del Hospital Universitario Materno Infantil de São Luís - MA. Se trata de un estudio cuantitativo, retrospectivo y transversal, basado en la recolección de datos proporcionados por la farmacia clínica, a través del análisis de las historias clínicas de los pacientes pediátricos hospitalizados, de los cuales se tomó la información de las intervenciones farmacéuticas registradas en la base de datos de Enero 2017 a diciembre 2018. Se obtuvieron 1003 intervenciones a través de la participación en visitas multidisciplinarias en la UCIP por parte de farmacéuticos del equipo de farmacia clínica. De estas 1003 intervenciones farmacéuticas, el 98% (n=983) fueron aceptadas y como las más prevalentes, en 2017, dosis/ajuste de dosis el 51% y la alternativa terapéutica más adecuada disponible el 25%. Así como, en el periodo de 2018, una caída de dosis/ajuste de dosis del 37%. Las intervenciones farmacéuticas contribuyeron a la reducción de errores relacionados con la farmacoterapia de los pacientes, contribuyendo a una mejora en la condición clínica, reducción de costos durante el período de tratamiento y hospitalización.A segurança do paciente tem sido tema de discussão nas instituições de saúde, principalmente nas Unidades de Terapia Intensiva Pediátrica, visto que os erros e suas consequências são mais graves em pacientes de cuidado intensivo. A intervenção farmacêutica, está dentro do acompanhamento farmacoterapêutico e requer registro, realizado em conjunto entre os profissionais de saúde e o paciente, com o objetivo de prevenir e solucionar resultados clínicos negativos. Assim, o objetivo deste trabalho foi avaliar o perfil das intervenções farmacêuticas realizadas pela farmácia clínica nos pacientes pediátricos internados na Unidade de Terapia Intensiva Pediátrica (UTIP) do Hospital Universitário Materno Infantil em São Luís – MA. Trata-se de um estudo quantitativo, retrospectivo e transversal, sendo feito a partir do levantamento dos dados fornecidos pela farmácia clínica, através das análises dos prontuários dos pacientes pediátricos internados, de onde foram retiradas as informações sobre as intervenções farmacêuticas registradas no banco de dados de janeiro de 2017 a dezembro de 2018. Obteve-se,1003 intervenções por meio da participação nas visitas multidisciplinares na UTIP pelos farmacêuticos da equipe da farmácia clínica. Destas 1003 intervenções farmacêuticas, 98% (n= 983) foram aceitas e como de maior prevalência, em 2017, ajuste de dose/ posologia 51% e alternativa terapêutica mais adequada disponível 25%. Como também, no período de 2018, uma queda de 37% no ajuste de dose/ posologia.
