Effects of antidepressant use during pregnancy: a systematic review

Resumo: Objective: To present and discuss research that analyzed the use of antidepressants in the gestational period and the effects found on the baby's development as a consequence of its exposure to pharmacological principles. Methodology: A bibliographic survey of 82 original publications was carried out, using descriptors: Pregnancy; Antidepressants; Embryonic and Fetal Development, in the VHL, Lilacs, Medline, Pubmed, Scielo, Science Direct, Scopus and Web of Science databases. After applying the inclusion and exclusion criteria proposed in the search design, a total of 9 publications accepted for use in this review. Results and Discussion: The results obtained indicate that there is clinical evidence that the use of antidepressants during pregnancy causes several effects that will compromise the baby's development, from the intrauterine period to the process of acquiring new skills in the initial period of life. However, the clinical picture of untreated depression (PDNT) in pregnant women has been associated with increased risks for miscarriages, premature births and low birth weight of babies. Research also reveals a significant presence in the high levels of cortisol in pregnant women, directly reflecting on fetal development and increased maternal irritability during the process. Final considerations: It was possible to show that using antidepressants in the gestational period is associated with risks for both the mother and the fetus, generating fetal development impairment such as: hypoglycemia, malformations related to the cardiovascular system, propensity to develop ASD, or other cognitive, behavioral and long-term, directly compromising the quality of life for both.

Metodología: Se realizó un levantamiento bibliográfico de 82 publicaciones originales, utilizando los descriptores: Embarazo; antidepresivos; Desarrollo Embrionario y Fetal, en las bases de datos BVS, Lilacs, Medline, Pubmed, Scielo, Science Direct, Scopus y Web of Science. Tras aplicar los criterios de inclusión y exclusión propuestos en el diseño de búsqueda, se aceptaron un total de 9 publicaciones para su uso en esta revisión. Resultados y Discusión: Los resultados obtenidos indican que existe evidencia clínica de que el uso de antidepresivos durante el embarazo provoca diversos efectos que comprometerán el desarrollo del bebé, desde el período intrauterino hasta el proceso de adquisición de nuevas habilidades en el período inicial de vida. Sin embargo, el cuadro clínico de depresión no tratada (PDNT) en mujeres embarazadas se ha asociado con un mayor riesgo de abortos espontáneos, nacimientos prematuros y bebés con bajo peso al nacer. Las investigaciones también revelan una presencia significativa en los altos niveles de cortisol en mujeres embarazadas, lo que se refleja directamente en el desarrollo fetal y aumenta la irritabilidad materna durante el proceso. Consideraciones finales: Se pudo evidenciar que el uso de antidepresivos en el período gestacional está asociado con riesgos tanto para la madre como para el feto, generando alteraciones en el desarrollo fetal tales como: hipoglucemia, malformaciones relacionadas con el sistema cardiovascular, propensión a desarrollar TEA, u otras alteraciones cognitivas, conductuales. ya largo plazo, comprometiendo directamente la calidad de vida de ambos.Objetivo: Apresentar e discutir pesquisas que analisaram o uso dos antidepressivos no período gestacional e os efeitos encontrados no desenvolvimento do bebê como consequência à sua exposição aos princípios farmacológicos. Metodologia: Realizou-se levantamento bibliográfico de 82 publicações originais, utilizando descritores: Gravidez; Antidepressivos; Desenvolvimento Embrionário e Fetal, nas bases de dados BVS, Lilacs, Medline, Pubmed, Scielo, Science Direct, Scopus e Web of Science. Após aplicação dos critérios de inclusão e exclusão propostos no delineamento de busca, totalizaram 9 publicações aceitas para utilização nesta revisão. Resultados e Discussão: Os resultados obtidos apontam que existem evidências clínicas de que o uso de antidepressivos durante a gravidez acarreta diversos efeitos que irão comprometer o desenvolvimento do bebê, desde o período intrauterino até seu processo de aquisição de novas habilidades no período inicial de vida. Contudo, o quadro clínico de depressão não tratada (DPNT) em grávidas vem sendo associado a riscos elevados para abortos espontâneos, partos prematuros e baixo peso no nascimento dos bebês. Pesquisas revelam ainda, presença significativa nos níveis elevados de cortisol nas gestantes, refletindo diretamente no desenvolvimento fetal e aumento da irritabilidade materna durante o processo.
