Physiotherapy treatment for pain reduction and improvement of function in jumping athletes with patellar tendinopathy: literature review

Resumo: Therapeutic exercises are widely used in the rehabilitation process of patellar tendinopathy (PT). However, there are divergences in the literature related to the intensity and/or frequency of these exercises. The aim of this study is to update and identify what would be the best kinesiotherapeutic intervention for the treatment of patellar tendinopathy (PT) to reduce pain and improve function in jumping athletes. The research is characterized as a literature review, following the PRISMA guidelines. The research was performed in the Pubmed and PEDro databases, articles published between the years 2005 and 2021, following the descriptors "Patellar tendinopathy" OR "jumper's knee AND treatment OR exercise therapy AND conservative treatment AND rehabilitation". The inclusion criteria were articles in English and randomized clinical trials. Studies in other samples, as well as articles that did not address the topic, were excluded. 221 studies were identified for this review, nine of which were selected for analysis and development. Therapeutic exercise through quadriceps muscle strengthening, regardless of type, was effective in reducing pain and improving function in athletes with PT. Among the forms of conservative treatment for PT, muscle strengthening is the most effective, safe, and long-lasting. However, regarding dose and intensity, the findings of this review are conflicting.

Sin embargo, existen divergencias en la literatura sobre la intensidad y/o frecuencia de estos ejercicios. El objetivo de este estudio es actualizar e identificar cuál sería la mejor intervención kinesioterapéutica para el tratamiento de la tendinopatía rotuliana con el fin de reducir el dolor y mejorar la función en los atletas de salto. La investigación se caracteriza por ser una revisión de la literatura, siguiendo las directrices PRISMA. La búsqueda se realizó en las bases de datos Pubmed y PEDro, artículos publicados entre 2005 y 2021, siguiendo los descriptores Patellar tendinopathy OR jumper's knee AND treatment OR exercise therapy AND conservative treatment AND rehabilitation. Los criterios de inclusión fueron artículos en inglés y ensayos clínicos aleatorios. Se excluyeron los estudios en otras muestras, así como los artículos que no abordaban el tema. Para esta revisión se identificaron 221 estudios, de los cuales se seleccionaron nueve para su análisis y desarrollo. El ejercicio terapéutico mediante el fortalecimiento de los músculos del cuádriceps, independientemente del tipo, fue eficaz para reducir el dolor y la función en los atletas con TR. Entre los tratamientos conservadores para la TR, el fortalecimiento es el más eficaz, seguro y duradero. Sin embargo, en cuanto a la dosis e intensidad, los resultados de esta revisión son contradictorios.Exercícios terapêuticos são amplamente utilizados no processo de reabilitação de tendinopatia patelar (TP). Entretanto, existem divergências na literatura entre qual intensidade e/ou frequência destes exercícios. O objetivo deste estudo é atualizar e identificar qual seria a melhor intervenção cinesioterapêutica para o tratamento de tendinopatia patelar para redução de dor e melhora da função em atletas saltadores. A pesquisa caracteriza-se como revisão de literatura, seguindo as orientações PRISMA. A busca foi realizada nas bases de dados Pubmed e PEDro, artigos publicados entre os anos de 2005 e 2021, seguindo os descritores Patellar tendinopathy OR jumper's knee AND treatment OR exercise therapy AND conservative treatment AND rehabilitation. Como critério de inclusão foram artigos na língua inglesa e do tipo ensaios clínicos randomizados. Foram excluídos estudos em outras amostras, bem como artigos que não abordassem o tema. Foram identificados 221 estudos para essa revisão, sendo nove selecionados para análise e desenvolvimento. O exercício terapêutico por meio do fortalecimento muscular de quadríceps, independentemente do tipo, foi efetivo para redução de dor e função em atletas com TP. Dentre as formas de tratamento conservador para TP, o fortalecimento é o mais eficaz, seguro e duradouro.
