Histamine-2 Receptor Antagonists and Proton Pump Inhibitors: Efficacy and drug prediletion in Stress Ulcer prophylaxis in patients admitted to the Pediatric Oncology ICU at Uopeccan Hospital

Resumo: Objective: To analyze and obtain results for Stress Ulcer prophylaxis with Proton Pump Inhibitors and Histamine-2 Receptor Antagonists in a patient in the pediatric ICU. The objective of this study reviews the predilection, existence of criteria and prophylactic effectiveness of both. Methods: It is developed from a retrospective observational study composed of medical records/data collection from patients in the Pediatric Intensive Care Unit at Hospital Uopeccan of Cascavel, between January 2012 and January 2022. Data relating to gender, age, diagnosis, reason for hospitalization, length of stay in the ICU and choice of gastric protector. Results: 43 cases of patients admitted to the ICU during the stipulated period were analyzed. Of these, the average age of patients in the research carried out was 4 years and 5 months, with 39.53% (n=17) female and 60.46% (n=26) male with an average hospitalization of approximately 10,6 days. Approximately 94.28% (n=33) of patients received some type of prophylactic, and within this percentage, 91.42% (n=32) of cases used PPI, 2.85% (n=1) ARH2 and 5.71% (n=2) did not use any medication. Conclusion: Based on this study, it is possible to conclude that there is a lack of scientific evidence and comprehensive research related to Proton Pump Inhibitors (PPI) and Histamine 2 Receptor Antagonists (ARH2) in clinical practice. This information gap may represent a potential disadvantage for patients, given the lack of specific criteria for Stress Ulcer prophylaxis in Pediatric Intensive Care Centers.

El objetivo de este estudio revisa la predilección, existencia de criterios y eficacia profiláctica de ambos. Métodos: Se desarrolla a partir de un estudio observacional retrospectivo compuesto por historias clínicas/recolección de datos de pacientes de la Unidad de Cuidados Intensivos Pediátricos del Hospital Uopeccan del Cascavel, entre enero de 2012 y enero de 2022. Datos relacionados a sexo, edad, diagnóstico, motivo de hospitalización, tiempo de estancia en UCI y elección del protector gástrico. Resultados: Se analizaron 43 casos de pacientes ingresados ​​en UCI durante el período estipulado. De estos, la edad promedio de los pacientes en la investigación realizada fue de 4 años y 5 meses, siendo 39,53% (n=17) del sexo femenino y 60,46% (n=26) del sexo masculino con una hospitalización promedio de aproximadamente 10,6 días. Aproximadamente el 94,28% (n=33) de los pacientes recibió algún tipo de profiláctico, y dentro de este porcentaje, el 91,42% (n=32) de los casos utilizó IBP, el 2,85% (n=1) ARH2 y el 5,71% (n=2) No utilizar ningún medicamento. Conclusión: Con base en este estudio, es posible concluir que existe una falta de evidencia científica e investigación integral relacionada con los inhibidores de la bomba de protones (IBP) y los antagonistas del receptor de histamina 2 (ARH2) en la práctica clínica. Esta brecha de información puede representar una desventaja potencial para los pacientes, dada la falta de criterios específicos para la profilaxis de las úlceras de estrés en los centros de cuidados intensivos pediátricos.Objetivo: analisar e obter resultados para profilaxia de úlcera de estresse com inibidores da bomba de prótons e antagonistas do receptor de histamina-2 em paciente na UTI pediátrica. O objetivo desse estudo revisa a predileção, existência de critérios e eficácia profilática de ambos. Métodos: desenvolve-se a partir de um estudo observacional retrospectivo composto por prontuários/coleta de dados de pacientes na unidade de terapia intensiva pediátrica no Hospital Uopeccan de Cascavel, entre janeiro de 2012 e janeiro de 2022. Foram avaliados dados referentes ao gênero, idade, diagnóstico, motivo do internamento, tempo de permanência na UTI e escolha do protetor gástrico. Resultados: foram analisados 43 casos de pacientes internados na UTI durante o período estipulado. Desses, a idade média dos pacientes da pesquisa realizada foi de 4 anos e 5 meses, sendo 39,53% (n=17) do sexo feminino e 60,46% (n=26) do sexo masculino com internação média aproximada de 10,6 dias. Cerca de 94,28% (n=33) dos pacientes recebeu algum tipo de profilático, sendo que, dentro dessa porcentagem, 91,42% (n=32) dos casos foi utilizado IBP, 2,85% (n=1) ARH2 e 5,71% (n=2) não foi utilizado nenhuma medicação. Conclusão: com base neste estudo, é possível concluir a carência de evidências científicas e de pesquisas abrangentes relacionadas aos inibidores de bomba de prótons (IBP) e antagonistas de receptores de histamina 2 (ARH2) na prática clínica.
