Dry needing in the treatment of myofascial pain associated with temporomandibular dysfunction: a systematic review

Resumo: | Myofascial pain is a muscle disorder that is often associated with temporomandibular disorders, affects the masticatory muscles and can lead to functional limitations. This systematic review aims to identify the scientific literature on the effects of superficial and deep dry needling for the treatment of pain in patients with myofascial temporomandibular disorder (TMD). This is a systematic review seeking primary studies in the PubMed and Virtual Health Library - VHL databases (combining databases such as LILACS - Latin American and Caribbean Literature in Health Sciences, MEDLINE - Medical Literature Analysis and Retrieval System Online, SciELO - Scientific Electronic Library Online, among others) with the descriptors “temporomandibular joint disorders dry needling”, or “myofascial temporomandibular dysfunction dry needling”, or “temporomandibular dysfunction dry needling”, as well as articles in English, Portuguese or Spanish that respond the guiding question of the use of dry needling in the treatment of myofascial TMD were included; theses, monographs, treatment associations, articles that were in languages other than those mentioned above, and systematic reviews were excluded. Nine studies were considered eligible. One study compared superficial and deep dry needling. Four studies used only deep dry needling and another 4 studies did not specify the type of needling used. Therefore, the results found suggest that there is not enough data to draw solid conclusions about which type of dry needling is best for the treatment of myofascial pain associated with TMD. Low risk of bias randomized controlled trials are strongly needed. |

Esta revisión sistemática tiene como objetivo identificar la literatura científica sobre los efectos de la punción seca superficial y profunda para el tratamiento del dolor en pacientes con trastorno temporomandibular miofascial (TTM). Se trata de una revisión sistemática que busca estudios primarios en PubMed y Virtual Health Library - Bases de datos BVS (que reúne bases de datos como LILACS - Literatura latinoamericana y caribeña en ciencias de la salud, MEDLINE - Medical Literature Analysis and Retrieval System Online, SciELO - Scientific Electronic Library Online, entre otros) con los descriptores “trastornos de la articulación temporomandibular punción seca”, o “disfunción temporomandibular miofascial punción seca”, o “disfunción temporomandibular punción seca”, así como artículos en inglés, portugués o español que respondan a la pregunta orientadora de la se incluyó el uso de punción seca en el tratamiento de TMD miofascial; Se excluyeron tesis, monografías, asociaciones de tratamiento, artículos en idiomas distintos a los mencionados anteriormente y revisiones sistemáticas. Se consideraron elegibles nueve estudios. Un estudio comparó la punción seca superficial y profunda. Cuatro estudios utilizaron solo punción seca profunda y otros cuatro estudios no especificaron el tipo de punción utilizado. Por lo tanto, los resultados encontrados sugieren que no hay datos suficientes para sacar conclusiones sólidas sobre qué tipo de punción seca es mejor para el tratamiento del dolor miofascial asociado con TMD. Se necesitan con urgencia ensayos controlados aleatorios de bajo riesgo de sesgo.Dor miofascial é um distúrbio muscular que esta frequentemente associada às desordens temporomandibulares, afeta os músculos da mastigação e pode levar a limitações funcionais. Esta revisão sistemática tem como objetivo identificar a literatura científica sobre os efeitos do agulhamento seco superficial e profundo para o tratamento da dor em pacientes com disfunção temporomandibular (DTM) miofascial. Trata-se de uma revisão sistemática buscando estudos primários nas bases de dados PubMed e Biblioteca Virtual em Saúde - BVS (que reúne bases de dados como LILACS - Literatura Latino-americana e do Caribe em Ciências da Saúde, MEDLINE - Medical Literature Analysis and Retrieval System Online, SciELO - Scientific Electronic Library Online, dentre outras) com os descritores “temporomandibular joint disorders dry needling”, or “myofascial temporomandibular dysfunction dry needling”, or “temporomandibular dysfunction dry needling”, assim artigos inglês, português ou espanhol que respondem à questão norteadora de utilização do agulhamento seco no tratamento de DTM miofascial foram incluídos; teses, monografias, associações de tratamentos, artigos que estavam em outras línguas que não as citadas acima e revisões sistemáticas foram excluídas. Nove estudos foram considerados elegíveis. Um estudo comparou o agulhamento seco superficial e profundo. Quatro estudos utilizaram apenas agulhamento seco profundo e outros 4 estudos não especificaram o tipo de agulhamento utilizado. Diante disso, os resultados encontrados sugerem que não há dados suficientes para tirar conclusões sólidas sobre qual tipo de agulhamento seco é melhor para o tratamento da dor miofascial associada à DTM.
