Resumo: | Chronic diseases have a gradual onset, with a long or uncertain duration and generally have multiple causes, covering a wide category of diseases and requiring reorganizations of the health system. The health care network of people with chronic diseases seeks to promote comprehensive care and expand actions to promote health and prevent the appearance of complications. This study aimed to analyze the access of people with chronic non-communicable diseases to health services, comparing the provision of care and procedures in two small municipalities in the Health Region 29, in Rio Grande do Sul (RS), with the parameters established by the Ordinance 1.631/2015. It is a cross-sectional and exploratory study, based on documents from the Municipal Health Secretariats and interviews with managers and key informants from two municipalities in RS. It was found that there is no difficulty in accessing health services in primary care; however, the offer of assistance in medium and high complexity is insufficient, hindering user access and inducing their financial participation in the cost of diagnosis and treatment. It was concluded that it is necessary to expand the supply of medium and high complexity care, with regard to chronic diseases, and it is up to the three federative entities to establish mechanisms to guarantee access to these services, as well as qualified regulation. |

La red de atención médica para personas con enfermedades crónicas busca promover la atención integral y ampliar las acciones para promover la salud y prevenir la aparición de complicaciones. Este estudio tuvo como objetivo analizar el acceso de personas con enfermedades crónicas no transmisibles a los servicios de salud, comparando la provisión de atención y procedimientos en dos municipios pequeños en la Región de Salud 29, en Rio Grande do Sul (RS), con los parámetros establecidos por la Ordenanza 1.631/2015. Es un estudio transversal y exploratorio, basado en documentos de las Secretarías de Salud Municipales y entrevistas con gerentes e informantes clave de dos municipios en RS. Se encontró que no hay dificultad para acceder a los servicios de salud en atención primaria; sin embargo, la oferta de asistencia en complejidad media y alta es insuficiente, lo que dificulta el acceso de los usuarios e induce su participación financiera en el costo del diagnóstico y el tratamiento. Se concluyó que es necesario ampliar la oferta de atención de complejidad media y alta, con respecto a las enfermedades crónicas, y corresponde a las tres entidades federativas establecer mecanismos para garantizar el acceso a estos servicios, así como una regulación calificada.As doenças crônicas apresentam início gradual, com duração longa ou incerta e geralmente têm múltiplas causas, contemplando uma categoria vasta de agravos e exigindo reorganizações do sistema de saúde. A rede de atenção à saúde das pessoas com doenças crônicas busca promover a atenção integral e ampliação das ações de promoção à saúde e de prevenção do aparecimento de complicações. Este estudo objetivou analisar o acesso das pessoas com doenças crônicas não transmissíveis aos serviços de saúde, comparando a oferta de atendimentos e procedimentos em dois pequenos municípios da Região de Saúde 29, do Rio Grande do Sul (RS), com os parâmetros estabelecidos pela Portaria 1.631/2015. Trata-se de um estudo transversal e exploratório, com base em documentos das Secretarias Municipais de Saúde e entrevistas com gestores e informantes-chave de dois municípios do RS. Verificou-se que não há dificuldade de acesso aos serviços de saúde na atenção básica; porém, a oferta de assistência na média e alta complexidade é insuficiente, dificultando o acesso do usuário e induzindo sua participação financeira no custeio do diagnóstico e tratamento.
