Sociodemographic and clinical profile of cancer patients in the context of the COVID-19 pandemic at a University Hospital in the Northeast: cross-sectional study

Resumo: | The COVID-19 pandemic has overloaded public health services, making it a barrier to administering anti-cancer therapies. The restructuring of health services and the vulnerability of cancer patients contributed to the interruption of anticancer treatment. Thus, the objective was to describe the sociodemographic and clinical profile and main outcomes of adult and elderly cancer patients admitted to a university hospital (HU), undergoing physical therapy intervention, considering the previous and current period of the pandemic. Exploratory and retrospective study involving 71 cancer patients admitted to the HU in 2019 and 2020. Epidemiological data were collected through electronic medical records. Eligibility criteria were registration of the primary ICD related to cancer and age group (≥ 30 and ≥ 60 years). The statistical analysis considered the confounding variables: age, gender, place of residence, type of cancer, clinical outcome (discharge or death), physical therapy approaches (oxygen therapy, invasive and non-invasive mechanical ventilation) length of stay. The results demonstrate that gender was not a determining variable for hospitalization. Higher incidences of hospitalization were observed in the elderly, African descendants, patients with cancer of the colon/rectum and rectum, lung and, in equal percentage, cancers of the breast, stomach and oropharynx. Patients who required physical therapy and oxygen therapy follow-up evolved the outcome of death, regardless of the type of intervention (invasive or non-invasive mechanical ventilatory support). There was difference between the number of admissions and discharge and death outcomes in 2020 when compared to 2019. |

La reestructuración de los servicios de salud y la vulnerabilidad de los pacientes con cáncer contribuyeron a la interrupción del tratamiento contra el cáncer. Así, el objetivo fue describir el perfil sociodemográfico y clínico y los principales desenlaces de los pacientes oncológicos adultos y ancianos ingresados en un hospital universitario (UH), sometidos a intervención de fisioterapia, considerando el período anterior y actual de la pandemia. Estudio exploratorio y retrospectivo en el que participaron 71 pacientes oncológicos ingresados en la UH en 2019 y 2020. Los datos epidemiológicos se recopilaron a través de historias clínicas electrónicas. Los criterios de elegibilidad fueron el registro del DAI primario relacionado con el cáncer y el grupo de edad (≥ 30 y ≥ 60 años). El análisis estadístico consideró las variables de confusión: edad, sexo, lugar de residencia, tipo de cáncer, resultado clínico (alta o muerte), enfoques de fisioterapia (oxigenoterapia, ventilación mecánica invasiva y no invasiva) duración de la estancia. Los resultados demuestran que el género no fue una variable determinante para la hospitalización. Se observaron mayores incidencias de hospitalización en ancianos, afrodescendientes, pacientes con cáncer de colon y recto, pulmón y, en igual porcentaje, cánceres de mama, estómago y orofaringe. Los pacientes que requirieron seguimiento de fisioterapia y oxigenoterapia evolucionaron al desenlace de muerte, independientemente del tipo de intervención (soporte ventilatorio mecánico invasivo o no invasivo). Assim, o objetivo foi descrever o perfil sociodemográfico, clínico e principais desfechos dos pacientes adultos e idosos com câncer internados em um hospital universitário (HU), submetidos à intervenção da Fisioterapia, considerando período prévio e atual à pandemia. Estudo exploratório e retrospectivo envolvendo 71 pacientes oncológicos internados no HU nos anos de 2019 e 2020. Os dados epidemiológicos foram coletados por meio de prontuários eletrônicos. Os critérios de elegibilidade foram registro do CID primário relacionado a neoplasia e faixa etária (≥ 30 e ≥ 60 anos). A análise estatística considerou as variáveis confundidoras: idade, sexo, local onde reside, tipo de neoplasia, desfecho clínico (alta ou óbito), condutas de fisioterapia (oxigenioterapia, ventilação mecânica invasiva e não invasiva) tempo de internação. Os resultados demonstram que o sexo não foi uma variável determinante para internação. Maior incidência de internação foram observados em idosos, afrodescendentes, pacientes com câncer de cólon e reto, pulmão e, em igual percentual, cânceres de mama, estômago e orofaringe. Os pacientes que necessitaram de acompanhamento fisioterapêutico e de oxigenoterapia evoluíram o desfecho óbito, independente do tipo de intervenção (suporte ventilatório mecânico invasivo ou não invasivo).
