Ruptured cornual ectopic of an advanced pregnancy: a case report

Resumo: Ectopic pregnancy is a pathology that presents a wide range of clinical manifestations, with numerous risk factors and a significant rate of complications. The pathophysiology refers to the implantation of the blastocyst. The most commonly reported clinical picture is related to vaginal bleeding with or without abdominal symptoms, but the characteristics can be variable because it is an embryonic implantation in the wrong place. There is a frequent association with pregnancies conceived through in vitro fertilization. Rarely, an ectopic pregnancy implants in the cornual region. Diagnosis is based on visualization of an extrauterine gestational sac with yolk sac or embryo, positive serum chorionic gonadotropin, and no products of conception on uterine aspiration or visualization on surgery. Treatment is related to the severity and symptoms presented by the individual. The objective of this study is to report a case of ruptured cornual ectopic pregnancy with advanced gestational age, showing the management and the form of treatment chosen for the patient.

La fisiopatología se refiere a la implantación del blastocisto. El cuadro clínico más comúnmente reportado se relaciona con sangrado vaginal con o sin síntomas abdominales, pero las características pueden ser variables por tratarse de una implantación embrionaria en el lugar equivocado. Existe una asociación frecuente con embarazos concebidos mediante fecundación in vitro. En raras ocasiones, un embarazo ectópico se implanta en la región cornual. El diagnóstico se basa en la visualización de un saco gestacional extrauterino con saco vitelino o embrión, gonadotropina coriónica sérica positiva y ausencia de productos de la concepción en la aspiración uterina o visualización en la cirugía. El tratamiento está relacionado con la gravedad y los síntomas que presenta el individuo. El objetivo de este estudio es reportar un caso de embarazo ectópico cornual roto con edad gestacional avanzada, mostrando el manejo y la forma de tratamiento elegido para la paciente.A gravidez ectópica é uma patologia que apresenta imensa gama de manifestações clínicas, com inúmeros fatores de risco e importante índice de complicações. A fisiopatologia é referente a implantação do blastocisto. O quadro clínico mais relatado é relacionado a sangramento vaginal com ou sem sintomas abdominais, porém as características podem ser variáveis por tratar-se de uma implantação embrionária em local errôneo. Nota-se frequente associação a gestações concebidas por fertilização in vitro. Raramente a gravidez ectópica implanta-se na região cornual. O diagnóstico é baseado na visualização de um saco gestacional extrauterino com saco de gema ou embrião, gonadotrofina coriônica sérica positiva e nenhum produto de concepção na aspiração uterina ou visualização na cirurgia. O tratamento está relacionado com a gravidade e os sintomas apresentados pelo individuo.
