Oral manifestations in pediatric patients with Acute Lymphoid Leukemia

Resumo: Objective: To review the literature on the main oral alterations found in pediatric patients with Acute Lymphoblastic Leukemia, whether caused by the disease or by antineoplastic treatment. Methodology: This is an integrative literature review that used the PubMed, Lilacs and Scielo databases through the keywords registered in the Health Sciences Descriptors (DeCS), in which studies that addressed the proposed theme were included. in this work, available in the aforementioned databases, in English and Portuguese, between the years 2011 to 2021. Results: 514 articles were found, of these 178 were initially screened based on titles and abstracts and only 34 articles met the eligibility criteria. Conclusion: Acute Lymphoblastic Leukemia, in most cases, leads to the appearance of oral lesions resulting from the disease, especially in the gingival regions, buccal mucosa, floor of the mouth, hard and soft palate. Chemotherapy and radiotherapy for Acute Lymphoblastic Leukemia are effective antineoplastic therapies, however, their side effects affect the oral cavity, with the most recurrent oral complications being: mucositis, candidiasis and radiation caries. It is of great value that the dentist is inserted in the multidisciplinary context, at the hospital and oncological level, as part of the success of the treatment of this leukemia, whether as a preventive or therapeutic approach.

Metodología: Se trata de una revisión integrativa de la literatura que utilizó las bases de datos PubMed, Lilacs y Scielo a través de las palabras clave registradas en los Descriptores de Ciencias de la Salud (DeCS), en la que se incluyeron estudios que abordaron la temática propuesta en este trabajo, disponibles en las bases de datos antes mencionadas, en inglés y portugués, entre los años 2011 a 2021. Resultados: se encontraron 514 artículos, de estos 178 fueron seleccionados inicialmente en base a títulos y resúmenes y solo 34 artículos cumplieron con los criterios de elegibilidad. Conclusión: La Leucemia Linfoblástica Aguda, en la mayoría de los casos, conduce a la aparición de lesiones orales derivadas de la enfermedad, especialmente en las regiones gingivales, mucosa bucal, piso de la boca, paladar duro y blando. La quimioterapia y la radioterapia para la Leucemia Linfoblástica Aguda son terapias antineoplásicas efectivas, sin embargo, sus efectos secundarios afectan la cavidad oral, siendo las complicaciones orales más recurrentes: mucositis, candidiasis y caries por radiación. Es de gran valor que el odontólogo se inserte en el contexto multidisciplinario, a nivel hospitalario y oncológico, como parte del éxito del tratamiento de esta leucemia, ya sea como enfoque preventivo o terapéutico.Objetivo: Revisar a literatura sobre as principais alterações bucais encontradas em pacientes pediátricos com Leucemia Linfoide Aguda, sejam elas causadas pela doença ou pelo tratamento antineoplásico. Metodologia: Trata-se de uma revisão de literatura integrativa que utilizou as bases de dados PubMed, Lilacs e Scielo por meio das palavras-chave cadastradas nos Descritores em Ciências da Saúde (DeCS), na qual, foram incluídos estudos que abordassem a temática proposta neste trabalho, disponíveis nas bases citadas, nas línguas inglesa e portuguesa, entre os anos de 2011 a 2021.  Resultados: Foram encontrados 514 artigos, destes 178 foram triados inicialmente a partir dos títulos e resumos e apenas 34 artigos atenderam os critérios de elegibilidade. Conclusão: A Leucemia Linfoide Aguda, na maioria dos casos, leva ao aparecimento de lesões orais decorrentes da doença, principalmente em regiões gengivais, mucosa jugal, assoalho de boca, palato duro e mole. A quimioterapia e a radioterapia para a Leucemia Linfoide Aguda são terapias antineoplásicas eficazes, porém, seus efeitos colaterais repercutem na cavidade bucal, sendo as complicações orais mais recorrentes: as mucosites, candidíase e cáries de radiação.
