Gap arthroplasty in a pediatric patient - case report

Resumo: | Ankylosis of the temporomandibular joint (TMJ) that occurs after fracture of the mandibular condyle constitutes a treatment challenge for oral and maxillofacial surgeons. It is a condition in which there is immobility of the joint, characterized by the formation of a bone, fibrous tissue or fibro-bone mass fused to the base of the skull. The most common factors for the progression of this condition are: trauma, systemic diseases and infections. The literature shows several ways of treatment such as: gap arthroplasty, interpositional gap arthroplasty using the temporal muscle, TMJ reconstruction using costochondral graft and alloplastic reconstruction of the joint. In view of this, this article aims to report the case of a child victim of trauma (cycling accident), diagnosed with a high fracture in the left mandibular condyle and to demonstrate the possibility of evolution of the fracture in ankylosis of the TMJ and, in sequence, to show the effectiveness of the surgical treatment performed by gap arthroplasty associated with physiotherapy as a successful protocol in the treatment of TMJ ankylosis in a pediatric patient with 2 years of follow-up. The data were included through the search for journals, scientific articles and course completion papers found in the Pubmed, BVS and Capes databases. |

Es una condición en la que hay inmovilidad de la articulación, caracterizada por la formación de un hueso, tejido fibroso o masa fibro-ósea fusionada a la base del cráneo. Los factores más comunes para la progresión de esta afección son: traumatismos, enfermedades sistémicas e infecciones. La literatura muestra varias formas de tratamiento, tales como: artroplastia de hueco, artroplastia de hueco interposicional usando el músculo temporal, reconstrucción de la ATM usando injerto costocondral y reconstrucción aloplástica de la articulación. En vista de esto, este artículo tiene como objetivo informar el caso de un niño víctima de un trauma (acidente de ciclismo), diagnosticado con una fractura alta en el cóndilo mandibular izquierdo y demostrar la posibilidad de evolución de la fractura en la anquilosis de la ATM y, en secuencia, mostrar la efectividad de la El tratamiento quirúrgico realizado por artroplastia gap asociado con la fisioterapia como un protocolo exitoso en el tratamiento de la anquilosis TMJ en un paciente pediátrico con 2 años de seguimiento. Los datos se incluyeron mediante la búsqueda de revistas, artículos científicos y trabajos de finalización de cursos que se encuentran en las bases de datos Pubmed, BVS y Capes.A anquilose da articulação temporomandibular (ATM) que ocorre após a fratura do côndilo mandibular constitui um desafio de tratamento para os cirurgiões buco-maxilo-faciais. É uma condição na qual existe imobilidade da articulação, caracterizada pela formação de um tecido ósseo, fibroso ou massa fibro-óssea fundida a base do crânio. Traumas, doenças sistêmicas e infecções estão entre os fatores mais comuns que desencadeiam a progressão desta condição. A literatura demonstra diversas maneiras de tratamento como: artroplastia em gap, artroplastia em gap interposicional usando o músculo temporal, reconstrução da ATM utilizando enxerto costocondral e reconstrução aloplástica da articulação. Diante disso, este artigo objetiva relatar o caso de uma criança vítima de trauma (acidente ciclístico), diagnosticada com fratura alta no côndilo mandibular esquerdo e demonstrar a possibilidade de evolução da fratura em anquilose de ATM e, em sequência, evidenciar a eficácia do tratamento cirúrgico realizado por artroplastia em gap associado a fisioterapia como protocolo de sucesso no tratamento de anquilose com 2 anos de acompanhamento. O englobamento dos dados se deu por meio da busca de em periódicos, artigos científicos e trabalhos de conclusão de curso encontrados nos bancos de dados Pubmed, BVS e Capes.
