Osseodensification in atrophic maxila for subsequente installation of dental implants

Resumo: The placement of implants in the maxilla has different characteristics from those found in the mandible. One of the most frequent challenges in the installation of implants is atrophy of the maxilla after partial or total loss of dental units. As a result, there is a significant decrease in the height and volume of the bone available in the posterior region of the maxilla. Currently, several techniques are used to improve bone quality and, consequently, the primary stability of the implant, from the established techniques such as Summers bone expansion technique, through osteotomes, and the most recent ones, such as osseodensification through Densah Bur® multilaminated drills, which consists of the condensation of the bone tissue walls made during preparation. The objective of the study was to present a clinical case using the osseodensification technique in the atrophic maxilla, for later installation of dental implants in the maxilla without using the Summers technique and bone grafts. After implant placement, Osstell® was used to assess primary stability. We conclude that osseodensification is a new biomechanical preparation technique that maintains the integrity of the alveolar ridge and allows the placement of implants with greater primary stability, allows for less morbidity and less time for completion of the rehabilitation treatment.

Uno de los desafíos más frecuentes en la instalación de implantes es la atrofia del maxilar después de la pérdida parcial o total de unidades dentales. Como resultado, hay una disminución significativa en la altura y el volumen del hueso disponible en la región posterior del maxilar. Actualmente, se utilizan varias técnicas para mejorar la calidad del hueso y, en consecuencia, la estabilidad primaria del implante, desde las técnicas establecidas, como la técnica de expansión ósea de Summers, hasta los osteótomos, y las más recientes, como la osteodensificación a través de fresas multilaminadas DensahÒ Bur, que consiste en la condensación de las paredes del tejido óseo durante la preparación. El objetivo del trabajo fue presentar un caso clínico utilizando la técnica de osteodensificación en el maxilar atrófico, para la posterior instalación de implantes dentales en el maxilar sin utilizar la técnica de Summers e injertos óseos. Después de la colocación del implante, se utilizó Osstell® para evaluar la estabilidad primaria. Concluimos que la osteodensificación es una nueva técnica de preparación biomecánica que mantiene la integridad de la cresta alveolar y permite la colocación de implantes con mayor estabilidad primaria, permite menos morbilidad y menos tiempo para completar el tratamiento de rehabilitación.A colocação de implantes na maxila apresenta particularidades diferentes das encontradas na mandíbula. Um dos desafios da instalação dos implantes mais freqüente é a atrofia da maxila após a perda parcial ou total das unidades dentárias. Como resultado há uma diminuição significativa na altura e volume do osso disponível na região posterior da maxila. Atualmente diversas técnicas são empregadas para melhora na qualidade óssea e consequentemente a estabilidade primária do implante, desde as consagradas como a técnica da expansão óssea de Summers, por meio de osteótomos, quanto as mais atuais como a osseodensificação através de brocas multilaminadas DensahÒ Bur, que consiste na condensação das paredes do tecido ósseo confeccionado durante o preparo. O objetivo do trabalho foi apresentar um caso clínico com a utilização da técnica de osseodensificação na maxila atrófica, para posterior instalação de implantes dentários na maxila sem a utilização da técnica de Summers e enxertos ósseos. Após a colocação dos implantes foi utilizado o Osstell® para avaliação da estabilidade primária.
