Phonoaudiological role in neurogenic dysphagia due to Alzheimer’s Disease

Resumo: Introduction: Swallowing is a biological function characterized as the act of swallowing and involves the participation of several muscles that depend on neurological organization for the proper functioning of this function. Neurological disorders can affect the coordinated contraction of the muscles involved in swallowing and cause dysphagia, as in Alzheimer's Disease (AD), a dementia that can occur in individuals of pre-senile or senile age. Given that individuals with Alzheimer's disease may present with Oropharyngeal Dysphagia (OD), the speech therapist is the professional capable of rehabilitating this dysphagia in order to promote food safety and the patient's quality of life. The objective of this work is to analyze speech therapy in neurogenic dysphagia caused by Alzheimer's disease. Methods: This is an integrative review that covered four databases, where a critical reading and analysis of studies published in the period between 2016 and 2022 was conducted, with the descriptors “Alzheimer's Disease” and “Swallowing Disorders”, “Alzheimer Disease” and “Swallowing Disorders”, in Portuguese and English. Results: From eleven articles, all research evaluated the clinical signs of oropharyngeal dysphagia in Alzheimer's disease, three addressed the relationship between speech therapy and AD, three aspects of speech therapy assessment and six addressed the rehabilitation of patients. Conclusion: The present study analyzed speech therapy in oropharyngeal dysphagia due to Alzheimer's disease and showed that the speech therapist is the professional capable of evaluating and treating swallowing disorders.

Los trastornos neurológicos pueden afectar la contracción coordinada de los músculos involucrados en la deglución y causar disfagia, como en la enfermedad de Alzheimer (EA), una demencia que puede ocurrir en individuos en edad presenil o senil. Dado que los individuos con enfermedad de Alzheimer pueden presentar Disfagia Orofaríngea (DO), el logopeda es el profesional capaz de rehabilitar esta disfagia con el fin de promover la seguridad alimentaria y la calidad de vida del paciente. El objetivo de este trabajo es analizar la logopedia en la disfagia neurogénica provocada por la enfermedad de Alzheimer. Métodos: Se trata de una revisión integradora que abarcó cuatro bases de datos, donde se realizó una lectura y análisis crítico de estudios publicados en el período comprendido entre 2016 y 2022, con los descriptores “Enfermedad de Alzheimer” y “Trastornos de la Deglución”, “Enfermedad de Alzheimer” y “Trastornos de la Deglución” en portugués e inglés.  Resultados: De once artículos, todas las investigaciones evaluaron los signos clínicos de la disfagia orofaríngea en la enfermedad de Alzheimer, tres abordaron la relación entre la logopedia y la EA, tres aspectos de la evaluación de la logopedia y seis abordaron la rehabilitación de los pacientes. Conclusión: El presente estudio analizó la logopedia en la disfagia orofaríngea por enfermedad de Alzheimer y demostró que el logopeda es el profesional capaz de evaluar y tratar los trastornos de la deglución.Introdução: A deglutição é uma função biológica caracterizada como o ato de engolir e tem a participação de diversos músculos que dependem da organização neurológica para um bom funcionamento desta função. Os distúrbios neurológicos podem afetar a coordenada contração dos músculos envolvidos na deglutição e ocasionar a disfagia, como na Doença de Alzheimer (DA), demência que pode ocorrer em indivíduos na idade pré-senil ou senil. Dado que, indivíduos com Doença de Alzheimer podem apresentar Disfagia Orofaríngea (DO), o fonoaudiólogo é o profissional apto para a reabilitação dessa disfagia no intuito de promover segurança alimentar e qualidade de vida do paciente. O objetivo deste trabalho é analisar a atuação fonoaudiológica nas disfagias neurogênicas por Doença de Alzheimer. Métodos: Esta é uma revisão integrativa que abrangeu quatro bases de dados, onde foi conduzida uma leitura e análise crítica dos estudos publicados no período entre 2016 e 2022, com os descritores “Doença de Alzheimer” e “Transtornos de Deglutição”, “Alzheimer Disease” and “Deglutition Disorders”, nos idiomas português e inglês. Resultados: A partir de onze artigos, todas as pesquisas avaliaram os sinais clínicos da disfagia orofaríngea na Doença de Alzheimer, três abordaram a relação da fonoaudiologia e DA, três os aspectos de avaliação fonoaudiológica e, seis abordaram a reabilitação dos pacientes.
