The impact of the telemedicine network on the epidemiology of congenital malformations in the state of Paraíba-Brazil: a comparison with worldwide incidences

Resumo: Congenital anomalies are malformations observed in prepartum or after birth. They significantly impact infant mortality rates and represent a public health problem. The study aims to evaluate the impact of the implementation of a fully Paraíba telemedicine network on local epidemiology related to congenital malformations, comparing them to worldwide incidences. It is a retrospective descriptive study, based on the records of births and malformations, for two years (2018 and 2019). The incidence of the study was accounted for by 11.815 patients, of which 476 presented malformation, while the continental incidences were collected from the literature. The statistical analysis of the data was performed by the T-Test and the One-way ANOVA test. There was a predominance of cardiac (30%), musculoskeletal (15%) and genito-urinary tract (13.9%) malformations. The statistical comparison of the malformations in Paraíba with the continental data reflected similarity with the Asian (p-value = 0,03037), African (p-value = 0,03327) and European (p-value = 0,04787) continents. The comparison of the population averages measured that the data of Paraíba, tied to the world data, is statistically within an acceptable pattern (p-value = 0,041638). The high incidence of malformations reported is possibly due to the local screening methods adopted associated with the telemedicine adopted by the Rede Cuidar. The similarity with developing continents denotes a reality of the impact of anomalies in these countries. The study was limited to the collection of continental data, related to the publication period of the selected studies and the grouped malformations.

Tienen un impacto significativo en las tasas de mortalidad infantil y representan un problema de salud pública. Para evaluar sus notificaciones, el estudio tiene como objetivo evaluar el impacto de la implementación de una red de telemedicina enteramente de Paraíba en la epidemiología local relacionada con las malformaciones congénitas, comparándola con las incidencias mundiales. Se trata de un estudio descriptivo retrospectivo, basado en los registros de nacimientos y malformaciones, durante dos años (2018 y 2019). La incidencia del estudio se contabilizó en 11.815 pacientes, de los cuales 476 tenían malformaciones, mientras que las incidencias continentales se recogieron de la literatura. El análisis estadístico de los datos se realizó mediante la prueba T y la prueba ANOVA. Predominaban las malformaciones cardíacas (30%), musculoesqueléticas (15%) y genitourinarias (13,9%). La comparación estadística de las malformaciones en Paraíba con los datos continentales reflejó similitud con los continentes asiático (p = 0,03037), africano (p = 0,03327) y europeo (p = 0,04787). La comparación de las medias poblacionales reveló que los datos de Paraíba, vinculados a los datos mundiales, están estadísticamente dentro de un estándar aceptable (p = 0,041638). La elevada incidencia de malformaciones señalada se debe posiblemente a los métodos de cribado locales adoptados asociados a la telemedicina adoptada por la Rede Cuidar. La similitud con los continentes en desarrollo denota una realidad del impacto de las anomalías en estos países. A incidência do estudo foi contabilizada por 11.815 pacientes, dos quais 476 apresentaram malformações, enquanto as incidências continentais foram coletadas da literatura. A análise estatística dos dados foi realizada pelo teste T e pelo teste ANOVA. Houve predominância de malformações cardíacas (30%), musculoesqueléticas (15%) e geniturinárias (13,9%). A comparação estatística das malformações na Paraíba com os dados continentais refletiu similaridade com os continentes asiático (p = 0,03037), africano (p = 0,03327) e europeu  (p = 0,04787). A comparação das médias da população revelou que os dados da Paraíba, ligados aos dados mundiais, estão estatisticamente dentro de um padrão aceitável (p = 0,041638). A alta incidência de malformações relatadas é possivelmente devido aos métodos locais de triagem adotados associados com a telemedicina adotada pela Rede Cuidar. A semelhança com os continentes em desenvolvimento denota uma realidade do impacto das anomalias nesses países.
