Experience of Balint Groups in the Medical Course: Impacts on the Relationship with the Patient and the Development of Empathic Skills

Resumo: | Introduction: It is an established fact that empathic skills are essential for building the therapeutic bond, a basic element in the doctor-patient relationship. The loss of empathic ability with a subsequent negative impact on resilience has been reported by medical students during the clinical phases of the course. Among the various interventions proposed for the development and facilitation of the empathic relationship are the Balint groups. Objective: To evaluate the participation in Balint groups in the development of an empathic attitude in the relationship established with the patient. Methods: This is an analytical, observational, cross-sectional study, in which two groups were compared: “Balint Group” and “Control Group”, to which the Wagnild & Young Resilience Scale and the Interpersonal Reactivity Index were applied. Results: Of the 36 participants, 8 were members of the Balint Group and 28 were from academic leagues. The Resilience Scale showed statistical superiority in the group of academic leagues, with a mean score of 115.6 ±19.3 obtained by participants in the Balint Group and 129.4 ±14.6 by members of academic leagues. As for the collection of the Interpersonal Reactivity Index, the Balint Group was statistically superior, with a mean score of 79±8.6 in the Balint Group and 64.4±13.6 in the academic leagues. Conclusion: The results support a more optimistic view on the issue of empathy and resilience in the medical curriculum and encourage the initiative of training aimed at helping young medical students to take into account the emotional component of the doctor-patient relationship. |

La pérdida de la capacidad empática con un impacto negativo posterior en la resiliencia ha sido reportada por estudiantes de medicina durante las fases clínicas del curso. Entre las diversas intervenciones propuestas para el desarrollo y facilitación de la relación empática se encuentran los grupos Balint. Objetivo: Evaluar la participación en los grupos Balint en el desarrollo de una actitud empática en la relación que se establece con el paciente. Métodos: Se trata de un estudio analítico, observacional, de corte transversal, en el que se compararon dos grupos: “Grupo Balint” y “Grupo Control”, a los que se les aplicó la Escala de Resiliencia de Wagnild & Young y el Índice de Reactividad Interpersonal. Resultados: De los 36 participantes, 8 eran miembros del Grupo Balint y 28 eran de ligas académicas. La Escala de Resiliencia mostró superioridad estadística en el grupo de ligas académicas, con una puntuación media de 115,6 ±19,3 obtenida por los participantes del Grupo Balint y de 129,4 ±14,6 por miembros de ligas académicas. En cuanto a la recogida del Índice de Reactividad Interpersonal, el Grupo Balint fue estadísticamente superior, con una puntuación media de 79±8,6 en el Grupo Balint y de 64,4±13,6 en las ligas académicas. Conclusión: Los resultados apoyan una visión más optimista sobre el tema de la empatía y la resiliencia en el currículo de medicina y alientan la iniciativa de formación dirigida a ayudar a los jóvenes estudiantes de medicina a tener en cuenta el componente emocional de la relación médico-paciente.Introdução: É fato estabelecido que as habilidades empáticas são essenciais para a construção do vínculo terapêutico, elemento básico na relação médico-paciente. Dentre as diversas intervenções propostas para o desenvolvimento e facilitação da relação empática estão os grupos Balint. Objetivo: Avaliar a participação nos grupos Balint no desenvolvimento de atitude empática na relação que se estabelece com o paciente. Métodos: Trata-se de um estudo analítico, observacional, corte transversal, em que foram comparados com dois grupos: “Grupo Balint” e “grupo controle”, aos quais foi aplicado a Escala de Resiliência de Wagnild & Young e o Interpersonal Reactivity Index. Resultados: Dos 36 participantes, 8 eram integrantes do Grupo Balint e 28 de ligas acadêmicas. A Escala de Resiliência demostrou superioridade estatística no grupo das ligas acadêmicas, com escore médio de 115.6 ±19.3 obtido pelos participantes do Grupo Balint e 129.4 ±14.6 pelos integrantes de ligas acadêmicas. Quanto a coleta do Interpersonal Reactivity Index, o Grupo Balint foi superior estatisticamente, com média em pontos de 79±8.6 no Grupo Balint e 64.4±13.6 nas ligas acadêmicas.
