Infection risks associated with care in prehospital care: impacts on patient safety

Resumo: Objectives: To identify incidents related to the risk of infection in land mobile pre-hospital care and to analyze the interfaces of this care with patient safety. Method: Observational, prospective study with evaluation of 239 occurrences. Data were analyzed using Epi-info and Statistical Package for Social Science statistical programs. The research was submitted to and approved by the ethics committee under registration number: 2,764,855. Results: 218 (92.8%) occurrences there was no hand hygiene between the procedures; gloves were not exchanged in 159 (69.1%) cases; hand hygiene was not performed before applying the dressing in 19 (46.3%) cases involving injuries; peripheral venipuncture was not performed using aseptic technique in 101 (60.8%) cases and in 06 occurrences aseptic bladder catheterization was not performed (85.7%). Conclusion: It is observed that mobile prehospital care is an environment of risk for infection associated with the care process. The results achieved in the research demonstrate that the objectives were achieved by presenting, quantitatively, the specific points that put the patient at risk with regard to their safety. Thus, it is necessary to invest in facilitating strategies for the proper performance of hand hygiene and control of the risk of infection in ambulance services, as well as the implementation of Patient Safety Centers strategically and operationally adapted to the mobile prehospital follow-up.

La investigación fue presentada y aprobada por el comité de ética con el número de registro: 2,764,855. Resultados: 218 (92.8%) ocurrencias no hubo higiene de manos entre los procedimientos; los guantes no se cambiaron en 159 (69,1%) casos; no se realizó la higiene de manos antes de aplicar el vendaje en 19 (46,3%) casos con lesiones; La venopunción periférica no se realizó mediante una técnica aséptica en 101 (60,8%) casos y en 06 casos no se realizó cateterización vesical aséptica (85,7%). Conclusión: se observa que la atención prehospitalaria móvil es un entorno de riesgo de infección asociado con el proceso de atención. Los resultados logrados en la investigación demuestran que los objetivos se alcanzaron presentando, cuantitativamente, los puntos específicos que ponen al paciente en riesgo con respecto a su seguridad. Por lo tanto, es necesario invertir en facilitar estrategias para el desempeño adecuado de la higiene de manos y el control del riesgo de infección en los servicios de ambulancia, así como la implementación de Centros de Seguridad del Paciente adaptados estratégica y operativamente al seguimiento prehospitalario móvil.Objetivos: Identificar incidentes relacionados ao risco de infecção no atendimento pré-hospitalar móvel terrestre e analisar as interfaces desse cuidado com a segurança do paciente. Método: Estudo observacional, prospectivo com avaliação de 239 ocorrências. Analisou-se os dados em programas estatísticos Epi-info e Statistical Package for Social Science. A pesquisa foi submetida e aprovada no comitê de ética sob número de registro: 2.764.855. Resultados: 218 (92,8%) ocorrências não houve higiene das mãos entre os procedimentos; não foi realizada a troca de luvas em 159 (69,1%) atendimentos; não foi realizada a higienização das mãos antes da aplicação do curativo em 19 (46,3%) atendimentos envolvendo lesões; não se realizou a punção venosa periférica dentro da técnica asséptica em 101 (60,8%) casos e em 06 ocorrências não foi realizada sondagem vesical de forma asséptica (85,7%). Conclusão: Observa-se que o atendimento pré-hospitalar móvel é um ambiente de riscos a infecção associada ao processo de cuidado. Os resultados alcançados na pesquisa demonstram que os objetivos foram alcançados ao apresentar, quantitativamente, os pontos específicos que colocam o paciente em risco no que tange sua segurança.
