Influence of antiretroviral therapy in oral manifestations of patients HIV+/AIDS

Resumo: | Human immune deficiency virus (HIV) infection is a pandemic (disease) characterized by severe changes of the imunne system that facilitated the appearance of opportunistic infections and neoplasias, could leading the patients to the development of the (when patients has developed full blown) Adquired Immune Deficiency Syndrome (AIDS). Oral manifestations of HIV (HIV+/AIDS) infection are one important factor that indicate the progress of the disease and occur in approximately 80% of infected patients. The aim of this work was to evaluate the influence of antiretroviral therapy in oral manifestations of patients HIV+/AIDS attended at the Hospital Dia - Itajaí (SC). Over one year period, 100 diagnosed HIV/AIDS patients that agreed to participate in this study were examined in a dentistry clinic and interviewed by trained and calibrated examiners, using a structured questionnaire. Clinical history was retrieved from patient's medical records. Results showed that 12 patients were in irregular treatment with anti-retroviral therapy (HAART), 63 in regular treatment and 25 were not receiving HAART. In all the three groups were found oral manifestations, and patients in regular treatment showed lesser cases. Oral candidiasis was the most common oral manifestation and was found in all the three groups. Patients in regular treatment showed CD4 count over than 200 cells/mm, and less viral load. |

A infecção pelo vírus da imunodeficiência humana (HIV) é uma pandemia caracterizada pela portabilidade de um fator que altera o sistema imunológico e facilita o aparecimento de infecções oportunistas e processos neoplásicos, quando o infectado passa a apresentar uma doença conhecida como síndrome da imunodeficiência adquirida (AIDS). As manifestações orais da infecção pelo HIV são parte importante da enfermidade e componentes indicativos de sua progressão. Até o aparecimento dos medicamentos antirretrovirais, cerca de 90% dos pacientes com AIDS tinham, pelo menos, uma manifestação oral. O objetivo deste trabalho foi avaliar o efeito da terapia antiretroviral na prevalência de manifestações orais em pacientes HIV+/AIDS em tratamento no Hospital Dia, em Itajaí (SC). A amostra de pacientes foi do tipo não probabilística constituída por conveniência e a obtenção de dados foi realizada através de entrevista com um questionário, análise de prontuários e exame clínico. Foram examinados 100 pacientes, dos quais 12 revelaram que não utilizavam diariamente sua terapia (uso irregular) com drogas antiretrovirais, 63 em tratamento regular (uso regular (diário) desta terapia) e 25 ainda não estavam em tratamento com este tipo de tratamento. Nos três grupos houve a presença de manifestações orais, e o grupo em tratamento regular foi o que apresentou menor número de pacientes com manifestações. A candidose oral foi a manifestação mais encontrada nos três grupos. Os pacientes em tratamento regular apresentaram alta taxa de células TCD4+ e baixa taxa de carga viral.
