TREATMENTS AND OUTCOMES IN INDIVIDUALS HOSPITALIZED AT JUQUERY HOSPITAL: DAILY LIFE IN THE VARGAS ERA

Resumo: Objective: To analyze the treatments and outcomes of patients hospitalized at Juquery Hospital from 1930 to 1945. Methods: Quanti-qualitative, exploratory-descriptive study, data analysis based on the documentary analysis framework. Data collection was carried out at the Historical and Cultural Heritage Collection of Juquery Hospital Complex, between March and July 2022. Data analysis also relied on historical documents in accordance with the social, political, ideological, economic, and cultural context in which the documents were produced and the authors were situated. Results: 2,166 medical records were analyzed; 920 (42.47%) records had no treatment data; 213 (9.83%) had no defined treatments. Main types of treatment: 494 (22.81%) monotherapy, with 196 (9.05%) using fever therapy, 157 (7.25%) using convulsive therapy, and 57 (2.63%) for general progressive paralysis/neurosyphilis. 235 (10.85%) underwent combination therapy, with 111 (5.12%) using fever therapy + anti-syphilis treatment and 73 (3.37%) using fever therapy + shock therapy. 2,005 (92.57%) medical records had no data on treatment effects. As for outcomes: 106 (4.89%) remained unchanged/did not improve, 21 (0.97%) improved, 18 (0.97%) worsened, and 16 (0.74%) experienced sudden death. There were 366 (16.90%) outcomes with no data, 868 (40.07%) deaths, of which 496 (57.15%) were unspecified, 263 (12.14%) discharges without clearance, 365 (16.85%) discharges with clearance, 36 (1.66%) evasions, and 252 (11.63%) transfers to other psychiatric institutions. Main causes of death: dysentery, malnutrition, anemia and cachexia, pneumonia/tuberculosis, cerebral hemorrhage/stroke. Conclusion: Treatments were mostly organic and did not yield effective results; outcomes indicate inadequacy and inefficiency in psychiatric care and hospitalization.

Métodos: Estudio cuantitativo-cualitativo, exploratorio-descriptivo, análisis de datos basado en el enfoque de análisis documental. La recolección de datos se realizó en la Colección del Patrimonio Histórico y Cultural del Complejo Hospitalario Juquery, entre marzo y julio de 2022. El análisis de datos también se basó en documentos históricos de acuerdo con el contexto social, político, ideológico, económico y cultural en el que se produjeron los documentos y en el que se encontraban los autores. Resultados: Se analizaron 2.166 historias clínicas; 920 (42,47%) historias clínicas sin datos de tratamientos; 213 (9,83%) sin definición de tratamientos. Los principales tipos de tratamiento fueron: 494 (22,81%) monoterapia, siendo 196 (9,05%) terapia febril, 157 (7,25%) terapia convulsiva y 57 (2,63%) para parálisis general progresiva/neurosífilis. Se realizaron 235 (10,85%) terapias combinadas, siendo terapia febril + antilues (111 - 5,12%), terapia febril + terapia de choque (73 - 3,37%); 2.005 (92,57%) historias clínicas sin datos sobre los efectos de los tratamientos. En cuanto a los resultados: 106 (4,89%) no presentaron cambios/no mejoraron, 21 (0,97%) mejoraron, 18 (0,97%) empeoraron y 16 (0,74%) fallecieron repentinamente. Hubo 366 (16,90%) resultados sin datos, 868 (40,07%) fallecimientos, de los cuales 496 (57,15%) no estaban especificados, 263 (12,14%) altas sin autorización, 365 (16,85%) altas con autorización, 36 (1,66%) evasiones y 252 (11,63%) traslados a otras instituciones psiquiátricas. Las principales causas de muerte fueron disentería, desnutrición, anemia y caquexia, neumonía/tuberculosis, hemorragia cerebral/ictus. Conclusión: Los tratamientos eran en su mayoría orgánicos y no producían resultados efectivos; los desenlaces indican inadecuación e ineficiencia en la atención psiquiátrica y la hospitalización.Objetivo: analisar os tratamentos e desfechos nos internados no Hospital do Juquery no período de 1930 a 1945. Métodos: estudo quanti-qualitativo, exploratório-descritivo, análise dos dados pelo referencial da análise documental. Coleta dos dados foi realizada no Acervo do Patrimônio Histórico-cultural do Complexo Hospitalar do Juquery, entre março e julho de 2022. Resultados: analisados 2.166 prontuários; 920 prontuários sem dados de tratamentos; 213 sem definição de tratamentos; principais tipos de tratamento: 494 monoterapia e 235 politerapia; 2.005 prontuários sem dados dos efeitos dos tratamentos. Quanto ao resultado: 106 inalterado/não melhorou, 21 melhorou, 18 piorou, 16 morte súbita. Houve 366 desfechos sem dados, 868 óbitos dos quais 496 não especificados, 263 saídas sem alta, 365 saídas com alta, evadidos 36 e 252 transferidos para outras instituições psiquiátricas.
