Cardiac surgery: strategies for coping with patients with surgical wound infection

Resumo: Background and Objectives: Surgical site infection is one of the adversities that can affect hospitalized patients after cardiac surgery. Each patient will react to it in a particular way, as well as develop coping mechanisms to get through the stressing event. This study aims to identify the predominant coping strategy in patients with surgical site infection after a cardiac surgery. Methods: Descriptive, cross sectional and quantitative studies, conducted between December 2017 and December 2018 at a cardiac referral hospital in the state of Rio Grande do Sul. The data collection was performed through a sociodemographic data questionnaire and applying the Ways of Coping Scale (Wocs). Results: The study included 33 post surgical patients with mean age of 62,4±8,8, of whom 69,7% had coronary artery bypass surgery, followed by surgical site infection, being chest infection the most prevalent (67%). The preponderant coping mechanism observed was the one based on the stressor (57,6%), followed by the one based on religious/fantasy thinking (27,3%), the one based on social support (15,2%) and the one based on emotion (0%). Conclusion: These data represent that patients with surgical site infection associated with cardiac surgery use coping that tends to be more adaptive and healthy, since their goal is to solve the problem.

Cada paciente reaccionará a este proceso de modo singular, así como, desarrollará estrategias de coping para atravesar el evento estresante. De esta forma, el objetivo del estudio fue identificar el modo de enfrentamiento en pacientes con infección de la herida operatoria asociados a la cirugía cardíaca. Métodos: Estudio descriptivo, transversal y cuantitativo realizado entre diciembre de 2017 a diciembre de 2018 en un hospital de referencia cardíaca en el estado de Rio Grande do Sul. La recolección de datos se realizó a través de un cuestionario de datos sociodemográficos y de la Escala de Modos de Enfrentamiento de Problemas (Emep). Resultados: Fueron evaluados 33 pacientes con edad de 62,4±8,8 años, de los cuales 69,7% realizaron la cirugía de revascularización del miocardio y tuvieron su herida operatoria infectada, siendo prevalente la infección de tórax (67%). El enfrentamiento emocional preponderante de estos pacientes fue el enfrentamiento con foco en el problema (57,6%), seguido de con foco en prácticas religiosas y pensamiento fantasioso (27,3%), con foco en búsqueda de soporte social (15,2%) y con foco en la emoción (0%). Conclusión: Estos datos indican que los pacientes con infección de la herida operatoria asociados a la cirugía cardíaca usan un enfrentamiento que tiende a ser más adaptativo y saludable, ya que su objetivo es resolver el problema.Justificativa e Objetivos: A infecção de sítio cirúrgico é um dos eventos adversos que pode acometer o paciente hospitalizado após a realização de uma cirurgia cardíaca. Cada paciente reagirá a este processo de modo singular, bem como, desenvolverá estratégias de coping para atravessar o evento estressante. Dessa forma, o objetivo do estudo foi identificar o modo de enfrentamento em pacientes com infecção da ferida operatória associados à cirurgia cardíaca. Métodos: Estudo transversal que avaliou o modo de enfrentamento emocional através da Escala de Modos de Enfrentamento de Problemas (Emep). Resultados: Foram avaliados 33 pacientes com idade de 62,4±8,8 anos, dos quais 69,7% realizaram a cirurgia de revascularização do miocárdio e tiveram sua ferida operatória infectada, sendo prevalente a infecção de tórax (67%). O enfrentamento emocional preponderante destes pacientes foi o enfrentamento focado no problema (57,6%), seguido de focado em práticas religiosas e pensamento fantasioso (27,3%), focado em busca de suporte social (15,2%) e focado na emoção (0%). Conclusão: Os resultados revelaram que os pacientes com infecção da ferida operatória utilizam predominantemente a estratégia focada no problema.
