Análise da penetração de H2O2 em dentes desmineralizados e submetidos a tratamento clareador

Resumo: | This study evaluated the H2O2 penetration and the evolution of demineralization in teeth with incipient lesions submitted to bleaching treatment. Among the selected 60 teeth, 30 had the enamel surface submitted to a cariogenic challenge in demineralizing solution and the rest remained sound. To analyze the H2O2 penetration, 15 enamel/dentin discs of each group received treatment with bleaching product based on hydrogen peroxide at 35%. During the procedure, the discs were placed in artificial pulp chambers containing acetate buffer solution. This solution was collected, processed, and submitted for analysis of optical density in Ultraviolet Visible reflectance spectrophotometer. Subsequently, three discs from each group were used for surface analysis with SEM and two others for the analysis of polarized light microscopy. The 10 remaining discs of each group were sectioned and polished for cross-sectional hardness analysis (CSH) at different depths. The data were submitted to ANOVA Repeated Measure and PLSD Fisher test at 5%. It was observed that previously demineralized teeth showed greater penetration of hydrogen peroxide (p <0.0001). After analyzing the CSH of the enamel, it was observed that bleaching caused changes to a depth of 20 μm in sound substrate, while in demineralized substrate the bleaching decreased the CSH of the enamel to a depth of 90 μm. It was concluded that the degree of the enamel mineralization could be a modulator of the bleaching agent's penetration in dental tissues and that bleaching agents contribute to the development of an incipient lesion's demineralization |

Dentre os 60 dentes selecionados, 30 tiveram a superfície do esmalte imersa em solução desmineralizadora e os restantes permaneceram hígidos. Para a análise da penetração de peróxido de hidrogênio, 15 discos de esmalte/dentina de cada grupo receberam tratamento clareador com o produto à base de peróxido de hidrogênio a 35%. Durante o procedimento, os discos foram posicionados em câmaras pulpares artificiais contendo solução tampão de acetato. Esta solução foi coletada, processada e submetida à análise da densidade óptica em Espectrofotômetro de Reflexão Ultravioleta Visível. Posteriormente, três discos de cada grupo foram destinados à análise superficial em MEV e outros dois para a análise de microscopia de luz polarizada. Os 10 discos restantes de cada grupo foram seccionados, polidos e destinados à análise da microdureza interna em diferentes profundidades. Os dados obtidos foram submetidos aos testes ANOVA medidas repetidas e Fisher PLSD ao nível de 5%. Foi observado que os dentes previamente desmineralizados apresentaram maior penetração de peróxido de hidrogênio (p<0.0001). Na análise da microdureza interna do esmalte, notou-se que o clareamento produziu alterações até a profundidade de 20 μm nos substratos hígidos, enquanto que no substrato desmineralizado, as alterações atingiram 90 μm.
