Hydroxyurea in the sickle cell anemia: toxicity and effectiveness

Resumo: Objective: to evaluate the use of hydroxyurea with regard to effectiveness and toxicity in people with sickle cell anemia. Method: this is a retrospective descriptive study, developed with 57 medical records of patients with sickle cell anemia, treated at the University Hospital Center of Campo Grande (Mato Grosso do Sul, Brazil), from 1993 to 2005. Inclusion criteria: electrophoresis of hemoglobin in medical record; regular use of drugs, for an average of 196 weeks; dosage; and hematological analyses before starting treatment. Exclusion criteria: living with other hemoglobinopathies. The variables evaluated were: neutrophils count; platelets; leukocytes; hemoglobin; time using hydroxyurea; drug response to the optimal dosage; and number and type of episodes of hospitalization. The research protocol was approved by the Ethics Committee of Universidade Federal de Mato Grosso do Sul, under the Protocol 645. Results: of the 57 medical records, 3 cases were evaluated. Comparing the hematological values, according to Portaria 872, enacted on 11/12/2002, it was found that: cases A, B, and C present an use of hydroxyurea (500 mg/day) for four years, with an average of 196 weeks. Case A, female, decreased painful episodes and frequency of hospitalization, keeping hematological values with no toxicity. In Case B, female, there was one hospitalization due to pain crises and important hemolysis. It stood out, in case C, male, neutropenia with hematological values < 2,000/mm3 . Conclusion: in the cases analyzed, we observed a drop in the number of hospitalizations with the decrease in painful crises from three to one a year, and there was no toxicity with regard to the dosage and time using hydroxyurea, in all three cases. For more comprehensive results, one suggests further study on this therapy with significant samples of this clientele.

Método: trata-se de estudo descritivo retrospectivo, desenvolvido com 57 prontuários de pacientes com anemia falciforme, atendidos no Núcleo de Hospital Universitário de Campo Grande (MS), de 1993 a 2005. Critérios de inclusão: eletroforese de hemoglobina registrada em prontuário; uso regular da droga, durante, uma média, 196 semanas; dosagem; e análises hematológicas antes de iniciar o tratamento. Critérios de exclusão: ser portador de outras hemoglobinopatias. As variáveis avaliadas foram: contagem de neutrófilos; plaquetas; leucócitos; hemoglobina; tempo de uso de hidroxiureia; resposta da droga à dosagem ideal; e número e tipo de episódios de internação. O protocolo de pesquisa foi aprovado pelo Comitê de Ética da Universidade Federal de Mato Grosso do Sul, sob o Protocolo n. 645. Resultados: dos 57 prontuários, três casos foram avaliados. Comparando os valores hematológicos, segundo a Portaria n. 872, de 12/11/2002, verificou-se que: os casos A, B e C apresentam uso de hidroxiureia (500 mg/dia), há quatro anos, com uma média de 196 semanas. O Caso A, sexo feminino, reduziu os episódios dolorosos e frequência de hospitalização, mantendo os valores hematológicos sem toxicidade. No Caso B, sexo feminino, registrou-se uma internação por crises de dor e hemólise importante. Destacou-se, no caso C, sexo masculino, neutropenia com valores hematológicos < 2.000/mm3 . Conclusão: nos casos analisados, observou-se redução das internações com a diminuição de crises dolorosas de três para uma ao ano, e não houve toxicidade quanto à dosagem e o tempo de uso da hidroxiureia, nos três casos. Para resultados mais abrangentes, sugerem-se estudos com essa terapia em amostras significativas dessa clientela.Objetivo: evaluar el uso de la hidroxiurea en cuanto a la eficacia y la toxicidad en personas con anemia de células falciformes. Método: esto es un estudio descriptivo retrospectivo que se llevó a cabo con 57 prontuarios de pacientes con anemia de células falciformes, tratados en el Centro Hospitalario Universitario de Campo Grande (Mato Grosso do Sul, Brazil), de 1993 hasta 2005. Criterios de inclusión: electroforesis de hemoglobina registrada en prontuario; el uso regular de la droga, durante un promedio de 196 semanas; dosificación; y análisis hematológicos antes de comenzar el tratamiento. Criterios de exclusión: ser portador de otras hemoglobinopatías. Las variables evaluadas fueron: recuento de neutrófilos; plaquetas; leucocitos; hemoglobina; tiempo de uso de la hidroxiurea; respuesta de la droga a la dosificación ideal; y el número y tipo de episodios de hospitalización. El protocolo de investigación fue aprobado por el Comité de Ética de la Universidade Federal de Mato Grosso do Sul, bajo el Protocolo 645. Resultados: de los 57 prontuarios, 3 casos fueron evaluados. Comparando los valores hematológicos, según la Portaria 872, de 12/11/2002, se constató que: los casos A, B y C presentan uso de hidroxiurea (500 mg/día), durante cuatro años, con una media de 196 semanas. El Caso A, sexo femenino, redujo los episodios dolorosos y la frecuencia de hospitalización, manteniendo los valores hematológicos sin toxicidad. En el caso B, sexo femenino, hubo una hospitalización por crisis de dolor y hemólisis importante. Se destacó, en el caso C, masculino, neutropenia con valores hematológicos < 2.000/mm3 . Conclusión: en los casos analizados, se observó una reducción en las hospitalizaciones con la disminución de crisis de dolor de tres para una al año, y no hubo toxicidad en cuanto a la dosificación y la el tiempo de uso de la hidroxiurea, en los tres casos.
